皮肤腺样囊性癌1例

摘　　要：患者女,59岁。左侧耳后斑块3年。皮肤科情况:左侧耳后见一直径约3.0cm×2.0cm的暗红色盘状浸润性斑块,界清,质硬,无压痛,凸出皮肤表面,上见少许鳞屑痂皮。皮损组织病理示:真皮中下部见大量由嗜碱性细胞构成的细胞巢,多数细胞巢中央可见管腔样结构,其内有嗜伊红均质样物质,细胞具有轻度异型性。免疫组织化学示:Bcl-2(+);CK(+);EMA(部分+);S100(个别核+);CEA(-);p63(-);SMA(-);Vimentin(-)。诊断:皮肤腺样囊性癌。建议患者外科手术切除联合放射治疗。A 59-year-old female presented with a plaque on the back of her left ear for 3 years. Cutaneous examination showed a dark red, well-circumscribed and nontender firm lesion on the back of left ear, and about 3.0 cm× 2.0cm in diameter,and it was on the surface with some scale and blood crust. Histopathology revealed normal epidermis,cell nests constituted by a lot of basophilic cells in the lower dermis, and most of the central cell nests formed luminal structure, within which is eosinophilic homogeneous substance. Immunohistochemistry showed positive reaction with Bel-2 and CK, weak positively for epithelial membrane antigen（EMA） and S100. Diagnosis：skin adenoid cystic carcinoma. The patient was referred to surgery to remove tumor tissue combined with radiotherapy.
