摘 要:目的探讨磁共振弥散张量成像(DTI)及纤维示踪技术(DTT)技术评估脑干肿瘤皮质脊髓束的应用价值。方法收集经手术病理证实的8例脑干肿瘤患儿,所有肿瘤均对脑干皮质脊髓束有不同程度的影响。男6例,女2例,年龄4~17岁,平均9岁;另收集与患儿性别及年龄基本匹配的8例正常对照儿童,年龄6~18岁,平均11岁,所有患儿及正常对照儿童均行DTI扫描。结果DTI及DTT技术能用于脑干肿瘤皮质脊髓束的损伤分级,所有病变与皮质脊髓束的位置关系均得到很好显示,术后所有患儿未出现新的神经系统症状,术前1例评级为中度缺失的患儿,术后评级为轻度缺失。结论DTI及DTT技术能直观显示脑干皮质脊髓束与肿瘤的位置关系,对术中保护皮质脊髓束及术后评估纤维束的恢复情况有重要的临床意义,并对手术方式的选择有一定的指导意义。Objective To explore the change of corticospinal tract (CST) on tensor imaging (DTI) and diffusion tensor tractography (DTT). Methods of brainstem tumors Eight cases of brain- stem tumors were analyzed. There were 6 males and 2 females with a mean age of 9 (4-17) years. An- other 8 normal controls were matched with regards to gender and age. All patients and normal controls underwent DTI scanning. Results Both DTI and DTT could be used to evaluation the status of CST and medial lemnicus by delineating the spatial relationship of tumors and fiber tracts. None of them had new symptoms of nervous system after surgery. Conclusions Capable of visualizing the relation- ship of tumors and fiber tracts, DTI and DTT are important for perioperative management and prog- nostic predication.
