西医脑出血与中医中风病病名诊断的对比研究

摘　　要：目的对比研究西医脑出血与中医中风病病名诊断的相关性。方法对438例脑出血患者临床资料进行回顾性分析,观察患者的临床症状、CT诊断结果及分型、中医临床诊断,分析2种诊断方法的相关性。结果 438例患者临床症状以半身不遂、口眼歪斜、舌强喎言、头痛及眩晕为主,出血部位以壳核区和丘脑多发,中医临床诊断以中脏腑为主。西医壳核出血、丘脑出血、脑叶出血、脑干出血均以中医中风病中脏腑为主,2种诊断均确诊者322例,均未确诊者52例。中医中风病病名诊断灵敏度为94.43%,特异性为53.61%,诊断符合率为85.39%,假阳性率为46.39%,似然比为2.035 6,X2=128.30,P<0.05。结论中医病名诊断联合西医诊断可对中风病定位、定性、定量诊断提供依据,从而提高中风病病名诊断符合率。Objective It is to compare and explore the correlation of western medicine cerebral hemorrhage and traditional Chinese medicine diagnosis of stroke. Methods 438 cases of patients with cerebral hemorrhage were selected and analyzed ret- rospectively, clinical symptoms, CT diagnosis results and typing, clinical diagnosis of traditional Chinese medicine of the pa- tients were observed and to analyze the correlation between this two kinds of diagnosis methods. Results The clinical symptoms of 438 patients mainly were hemiplegia, facial paralysis, stiff tongue and sluggish speech, headache and vertigo, putamen and thalamus were the common hemorrhage position, TCM clinical diagnosis mainly was attack on viscera. Western medicine puta- men hemorrhage, thalamic hemorrhage, lobar hemorrhage, brain stem hemorrhage mainly were attack on viscera of traditional Chinese medicine stroke. In the patients, 322 cases＇s two kinds of diagnosis were confirmed, 52 cases were not diagnosed. Stroke disease diagnostic sensitivity was 94.43% , specificity was 53.61% , the diagnostic accordance rate was 85.39% , false positive rate was 46.39% , the likelihood ratio was 2. 035 6, χ2 = 128.30, P 〈 0.05. Conclusion TCM disease name diagno- sis combined with western medicine diagnosis can provide evidences for stroke positioning, qualitative, quantitative diagnosis, thus to increase the diagnostic rate of stroke disease name.
