64层螺旋CT血管成像在肠系膜血管缺血性疾病中的临床价值

摘　　要：目的 探讨64层螺旋CT在肠系膜血管缺血性疾病中的价值.方法 回顾性分析17例临床怀疑肠缺血行肠系膜血管CTA检查的患者资料,并与手术、病理结果及出院诊断对照.以出院诊断作为标准,将17例患者分为肠缺血组和对照组,分析2组患者间CTA征象的差异.结果 肠缺血组中肠系膜上动脉栓塞1例,肠系膜上动脉血栓4例,其中1例合并肠系膜上静脉血栓,肠系膜上动脉硬化致管腔重度狭窄2例,肠系膜上静脉血栓1例.对照组中2例表现为肠系膜上动脉及肠系膜上静脉扭转.17例患者中肠管积液11例,肠壁增厚6例,肠腔积气7例,腹水4例.结论 64层螺旋CT能够清晰显示肠系膜血管缺血的直接征象与间接征象,并有助于外科术前指导和内科疗效评价.Objective To explore the value of 64-slice spiral CT and post-processing technology in the mesenteric vascular ischemic diseases.Methods 17 cases of patients with suspected intestinal ischemia was retrospective analyzed.CTA findings were correlated with surgery,pathology and final diagnosis.Patients were divided into two groups：a study group of patients with intestinal ischemia and a control group with an alternate diagnosis,for the purpose of statistical analysis.Results Among the group with intestinal ischemia,1 case showed embolism in superior mesenteric artery （SMA）,4 cases showed thrombosis in SMA,of which 1 case combined with thrombosis in superior mesenteric vein（SMV）,2 cases showed severe stenosis in SMA,1 case showed thrombosis in SMV.Among the control group,2 cases showed torsion in SMA and SMV.Among 17 patients,11 cases showed insufflate effusion,6 cases showed bowel wall thickening,7 cases showed pneumatosis,4 cases with ascites.Conclusion 64-slice spiral CT can demonstrate direct and indirect signs of mesenteric vascular ischemia clearly,which is effective for the surgery preoperative guidance and evaluation of medical treatment.
