超声造影诊断原发性肝癌术后肝内复发转移的价值

摘　　要：目的探讨超声造影在原发性肝癌术后肝内复发转移的诊断价值。方法收集我院76例原发性肝癌术后肝内复发转移患者的影像学资料,对比分析超声造影、彩色多普勒超声及增强CT诊断复发转移灶的结果。结果彩色多普勒发现复发转移灶51个,增强CT发现复发转移灶77个,超声造影发现复发转移灶82个,病灶检出率彩色多普勒超声与超声造影差异有显著性意义(X^2=26.8,P<0.000 1),增强CT与超声造影差异无显著性意义(X^2=1.24,P>0.05),但对长径<1 cm的病灶超声造影比增强CT具有更高的灵敏度(X^2=3.86,P<0.05)。结论超声造影对原发性肝癌术后复发转移的诊断具有重要临床价值,可以作为肝癌术后随诊的可靠影像学方法。Objective To explore the value of contrast-enhanced ultrasonography for postoperative intrahepatic recurrence and metastasis in hepatocellular carcinoma.Methods 76 cases of postoperative patients with Primary hepatocellular carcinoma were followed up,the result of contrast-enhanced ultrasonography,color Doppler ultrasound and enhanced-CT diagnosis of metastatic recurrence were compared and analyzed.Results 51 recurrent metastases were found by color Doppler ultrasound,77 recurrent metastases were found by enhanced-CT,82 recurrent metastatic lesions were found by contrast-enhanced ultrasonography,in terms of lesion detection rate,the differences of color Doppler ultrasound and contrast-enhanced ultrasonography were significant （x2=26.8,P＜0.000 1）,enhanced CT and contrast-enhanced uhrasonography were unsignificant （x2=1.24,P＞0.05）.Of the lesions lesser than 1 cm,the sensitivity of contrast-enhanced ultrasonography is higher than that in enhanced CT （x2 =3.86,P＜0.05）.Conclusions Contrast-enhanced ultrasonography has important clinical value in the diagnosis of postoperative intrahepatic recurrence and metastasis in primary hepatocellular carcinoma,it＇s a reliable method to detect the intrahepatic recurrence and metastatic lesions of hepatocellular carcinoma during postoperative follow-up.
