乳腺叶状肿瘤的MRI表现与病理特征观察

摘　　要：目的:对乳腺叶状肿瘤的MRI表现分析。方法:对手术病理证实的12例乳腺叶状肿瘤MRI表现进行回顾性分析,包括平扫信号强度、形态学、增强病变内部强化特征的扫描、时间-信号强度曲线类型、ADC表现扩散系数值及DWI信号值。对正常乳腺组织ADC与乳腺叶状肿瘤PTs进行配对设计资料t的检验比较。结果:术后病理诊断12例乳腺叶状肿瘤PTs,其中良性7例,交界性4例,恶性1例。MRI上5例为椭圆形,7例为分叶状;4例欠清,8例边界清晰;12例乳腺叶状肿瘤PTs,MRI平扫TIWI过后,呈现低信号;STIR或T2WI脂肪抑制呈现高信号,其中3例肿瘤内见囊性极高不规则信号影,3例见低信号分隔;乳腺叶状肿瘤PTs在初期呈动态快速增强状态,初始期后TIC9例呈II型,3例呈I型;DWI检查11例,其表现均为高信号,正常乳腺组织ADC值与患者平均ADC值相比有显著差异,具有统计学意义(p<0.05)。结论:乳腺叶状肿瘤PTs的MRI表现特点无明显特征,因此为非特征性,需经过组织病理学检查确诊。Objective：This study was conducted to analyze the phyllodes tumor of the breast MRI. Materials：and Methods 12 pa- tients with pathologically confirmed breast phyllodes tumor MRI findings were retrospectively analyzed, including the unenhanced signal intensity , morphology, characterized by enhanced internal lesions enhanced scan time - signal intensity curve type, ADC performance diffusivity value and DWI signal values. ADC normal breast tissue and breast phyllodes tumor PTs tested design data were compared with paired t~. Results： Among 12 cases of postoperative pathological diagnosis of breast phyllodes tumor PTs, seven cases were be- nign, four cases of borderline and malignant one case. On MRI 5 cases oval, seven cases lnhular; 4 cases less clear, eight cases clear boundaries; 12 cases of breast phyllodes tumor PTs, after the MRI scan TIWI, showed low signal; STIR or fat - suppressed T2WI showed high signal, wherein the three cases of cystic tumors see extremely irregular signal intensity, three cases are low signal separator ; PTs in early breast phyllodes tumor was rapidly increasing dynamic state, after an initial period TIC 9 cases were type II, 3 cases were type I; DWI was used to examin 11 cases, all of which are high - performance signal. ADC values of normal breast tissue in pa- tients with mean ADC values have significant differences compared with statistical significance（ p 〈 0.05 ）. Conclusion： Phyllodes tumor PTs MRI features shew no obvious features, therefore they are non -characteristic and should be taken histopathological examination.
