摘 要:目的探讨帕金森病睡眠障碍的结构特点及影响因素。方法应用多导睡眠监测仪和匹茲堡睡眠质量指数问卷(PSQI)对15例帕金森病患者(PD组)和18例年龄、性别相匹配的健康成人(对照组)行多导睡眠监测和睡眠质量评估。结果两组比较,睡眠时间、睡眠效率、3+4期睡眠时间、3+4期睡眠比、动眼睡眠期(REM)时间、2期睡眠比、睡眠潜伏期差异均具有统计学意义(P均<0.05)。睡眠破碎、慢波睡眠缺失发生率PD组分别为73.3%、80.0%,对照组分别为33.3%、22.2%,两组比较差异具有统计学意义(P均<0.05)。逐步多元回归分析示PD组PSQI得分与左旋多巴剂量和病程呈线性相关(t分别为2.912、2.992,P均<0.05)。结论帕金森患者存在多种睡眠结构的改变,睡眠质量与病程、H-Y分级、左旋多巴剂量有关。Objective To investigate the architectures,characteristics and influence factors of sleep disturbance in patients with Parkinson disease (PD).Methods Fifteen patients with PD (PD group) and 18 healthy persons of sexand age-matched as the controls (control group) were monitored by using polysomnography (PSG),and were evaluated by Pittsburgh Sleep Quality Index (PSQI),respectively.Results There were statistical differences in total sleep time,sleep efficiency,the sleep time of stage 3 + 4,percentage of stage 3 + 4,sleep time of REM,percentage of stage 2 and sleep latency between the PD group and control group (all P < 0.05).The prevalence of sleep fragmentation and slow wave sleep (SWS) deletion in the PD group was respectively 73.3% and 80.0%,while 33.3% and 22.2% in the control group (all P < 0.05).Multivariate linear regression analysis showed that PQSI total score was significantly associated with doses of Ldopa daily and duration of disease (t =2.912,2992 ; all P < 0.05).Conclusion The sleep architectures change commonly in patients with PD.Meanwhile,duration of disease,doses of L-dopa daily,Hoehn and Yahr stage are significantly related to sleep quality.
