摘 要:目的探讨胰岛素生长因子1受体(insulin-like growth factor 1 receptor,IGF-1R)、IGFBP-3在结直肠癌(colorectal cancer,CRC)发病机制中的作用和临床意义。方法应用免疫组化SP法检测183例CRC组织和61例正常结直肠黏膜中IGF-1R、IGFBP-3的表达。结果 (1)IGF-1R在CRC中的阳性率(69.4%)显著高于对照组(26.2%),P<0.001;IGFBP-3在CRC中的阳性率(78.1%)显著低于对照组(95.1%),P=0.003;(2)IGF-1R在CRC中的表达与肿瘤浸润深度、TNM分期、淋巴结转移、远处转移均呈正相关(P<0.05);IGFBP-3在CRC中的表达与CRC TNM分期、淋巴结转移均呈负相关(P<0.05),与其它因素无关(P>0.05);(3)IGF-1R与IGFBP-3的表达呈负相关(P=0.005)。结论 IGF-1R和IGFBP-3在CRC中的表达与多项临床病理参数具有相关性。联合检测IGF-1R、IGFBP-3有望成为CRC评估预后的生物学指标,为CRC的治疗提供新靶点。Purpose To investigate the roles and the clinical significance of IGF-1R and IGFBP-3 in the pathogenesis of colorectal cancer (CRC). Methods The expression of IGF-1R and IGFBP-3 was detected in 183 cases of CRC tissues and 61 cases of normal colorectal mucosa tissues by SP immunohistochemieal technology. Results ( 1 ) The IGF-1R positive expression rate of 69.4% in CRC was significantly higher than that of 26. 2% in control group ( P 〈 0. 001 ). The IGFBP-3 positive expression rate of 78. 1% in CRC was significantly lower than that of 95.1% in control group ( P = 0. 003 ). ( 2 ) There were positive correlations between the IGF- 1R expression and these clinical pathological parameters in CRC, including the depth of invasion, TNM stage, lymph node metastasis, and distant metastasis ( P 〈 0. 05 ). The IGFBP-3 expression in CRC showed negative correlations to the TNM stage and lymph node metastasis ( P 〈 0. 05 ), independent of other factors ( P 〉 0. 05 ). ( 3 ) The expression of IGF-1 R and IGFBP-3 was negatively correla- ted (P = 0. 005 ). Conclusion There are correlations between the expression of IGF-1R and IGFBP-3 in CRC and various clinical pathological parameters in CRC. Combined detections of IGF-1R and IGFBP-3 could become new biomarkers in assessment of CRC prognosis, and might supply a new target to the therapeutic use for CRC.
