吉兰-巴雷综合征肌电图的感觉分离现象

摘　　要：目的探讨吉兰-巴雷综合征(GBS)患者肌电图的感觉分离现象。方法回顾性分析20例肌电图存在感觉分离现象的吉兰-巴雷综合征患者。20例均进行肌电图(EMG)、神经传导速度(MCV、SCV)、F波检测。检测双侧正中神经、尺神经、腓总神经、胫神经的运动传导速度,双侧正中神经、尺神经、腓肠神经的感觉传导速度以及双侧尺神经、胫神经F波的潜伏期及出现率。针极肌电图检测单侧外展拇短肌、肱二头肌、胫前肌和股四头肌。结果 20例确诊为吉兰-巴雷综合征的患者肌电图检查出现:双侧腓肠神经感觉传导速度均在正常范围,19例双侧或单侧或单条正中神经、尺神经感觉传导速度减慢或波幅减低,1例双正中、尺神经感觉传导速度未引出明显反应。结论在临床表现典型的吉兰-巴雷患者肌电图出现了这种感觉分离现象更支持诊断。神经电生理的检查对吉兰-巴雷综合征的诊断具有重要的参考价值。Objective Tb explore the dissociated sensory disturbance showed by electromyography （EMG） in patients with Guillain-Barre syndrome（GBS）. Methods The clinical data of 20 cases of GBS with dissociated sensory disturbance showed by EMG were analyzed retrospectively. EMG, motor nerve conduction velocity （MCV） , sensory nerve conduction velocity（SCV） ,and F-wave conduction velocity（FWCV） were measured in all cases. MCV and SCV of the bilateral median nerve, ulnar nerve, common peroneal nerve and tibial nerve were measured; F-wave latency and frequency of bilat eral ulnar nerve and tibial nerve were examined;needle electromyography waperformed to study the unilateral abductor pollicis brevis muscle, biceps brachii, tibialis anterior and quadriceps. Results SCV of double lateral Sural nerve in 20 cases of GBS was in the normal range;the lower SCV or reduced amplitude were found in bilateral/unilateral/single median nerve and unilateral in 19 cases ; SCV of bilateral median nerve and unilateral did not changed in 1 case. Conclusion The dissociated sensory disturbance showed by EMG in GBS patients with typical clinical manifestations should confirmed the diagnosis of GBS. The EMG detection might be the very important method for diagnosing GBS.
