摘  要:目的通过3D-CTA和3D-DSA临床应用,探讨二种血管成像技术在脑动脉瘤介入栓塞治疗中的应用价值。方法收集51例因脑动脉瘤破裂致蛛网膜下腔出血患者行3D-CTA和3D-DSA检查,并全部行血管内介入治疗。结果 51例患者中,CTA检查发现52个动脉瘤,DSA发现53个动脉瘤,二种检查在准确性和灵敏性上无显著差异。53个动脉瘤全部行介入栓塞治疗,栓塞程度0级45个、1级8个。结论 3D-CTA检查无创、经济、快捷、简便,可作为蛛网膜下腔出血的患者首选检查方法;3D-DSA对重要穿支血管及动脉瘤瘤颈情况显示优势,可作为可疑复杂性动脉瘤的对比筛查方法,并且可行血管内介入治疗。两种技术互相补充,完善脑动脉瘤的检查与治疗。Objective To evaluate the application of 3D-CTA and 3D-DSA imaging in the endovascular treatment for cerebral aneurysms(CAs). Methods Fifty-one patients with subarachnoid hemorrhage of ruptured cerebral aneurysm were all con firmed by 3D-CTA and 3D-DSA. All patients received interventional endovascular treatment. Results Fifty-two aneurysms in 3D-CTA imaging were found, and 53 aneurysms in 3D-DSA imaging were found. The specificity and sensitivity of 3D-CTA and 3D-DSA had no significant difference. 45 of 53 CAs were completely embolized, 8 CAs were almost completely embolized. Con- clusion 3D-CTA is a non-traumatic, economical, rapid and convenient method in cerebral blood vessel imaging, so it can be es- pecially used for the first choice of detection method in spontaneous subarachnoid hemorrhage. 3D-DSA is superior in showing important perforating branches of the aneurysm and the blood direction of instant aneurysm-bearing artery, so it can be the con- trasting screening method of suspicious complicated aneurysm. Both techniques can supplement for each other in detection and therapy of cerebral aneurysm.
