摘  要:目的 研究H ras基因在乳腺癌转移中的作用及作为预测乳腺癌转移潜能指标的价值。方法 采用免疫组织化学LSAB法观察 76例乳腺癌、34例淋巴结转移灶和 8例乳腺纤维腺瘤组织中 p2 1蛋白、PCNA、CD44、ⅧRAg的表达情况 ;采用PCR RFLP法检测 42例乳腺癌、6例乳腺纤维腺瘤H ras 12位密码子点突变状态。结果  76例乳腺癌 p2 1蛋白阳性 5 8例 ,有淋巴结转移组阳性率明显高于无转移组 ,2组比较有显著性差异 (P <0 .0 5 )。p2 1蛋白与CD 44、PCNA表达均呈正相关 (γ =0 .6 337,P <0 .0 1;γ =0 .30 44,P <0 .0 5 )。p2 1蛋白阳性组肿瘤间质血管密度 (IMVD )明显高于阴性组 ,2组比较有非常显著性差异 (P <0 .0 1)。结论 p2 1蛋白表达与乳腺癌淋巴结转移密切相关 ;与CD 44、IMVD联合检测可作为预测乳腺癌转移潜能敏感且准确的指标。Objective To investigate the value of H ras oncogene as metastatic potential marker in breast cancer.Methods LSAB method was used to detect the expression of p21、CD44、PCNA、ⅧRAg in 76 cases of breast cancer,34 cases of lymphnode metastastic tissue,8 cases of breast fibroadenoma.The mutation at codon 12 was analyzed by PCR RFLP method in 42 cases of breast cancer and 6 cases of breast fibroadenoma.Results The total positive rate of p21 protein expression was 76.3%(58/76) in breast cancer,in node positive group it was significantly higher than that in node negative one( P <0.05).The expression of p21 had a positive relativity with the expression of CD44( P <0.01) and PCNA( P <0.05).IMVD was higher in p21 positive group than that in p21 negative one( P <0.01).Conclusion p21 protein expression has markedly correlation with metastasis.Detection of p21 combined to CD44 and IMVD will be a sensitive marker for predication of metastatic potential in breast cancer.
