多指标评价中西医治疗小细胞肺癌疗效

摘　　要：目的采用中西医不同的评价方法结合小细胞肺癌血清肿瘤标志物Pro-GRP和NSE评价"养阴润肺协定方"在治疗小细胞肺癌中的疗效。方法收集首诊病例71例小细胞肺癌,包括局限期30例和广泛期41例。按治疗方法不同分两组,单化组33例,综合组38例。在第3次化疗前(42 d),根据中医治疗中晚期肺癌的临床受益评定标准,WHO抗癌药毒性反应分级标准和实体瘤疗效评价标准(RECIST),对患者临床症状、生存质量、毒副反应及瘤体疗效进行评价,并对患者血清肿瘤标志物Pro-GRP和NSE进行跟踪检测。结果按照实体瘤疗效评定标准(RECIST)评价,两组疗效差异无统计学意义(P>0.05);按照中医评定标准和药物毒性标准评价,综合组在改善临床症状、提高生存质量、减轻毒副反应方面都优于单化组(P<0.05)。综合组患者在6次化疗后血清肿瘤标志物Pro-GRP和NSE的下降趋势比单化组更明显(P<0.05)。结论 "养阴润肺协定方"在治疗小细胞肺癌方面具有增效减毒作用,血清肿瘤标志物Pro-GRP和NSE是监测小细胞肺癌疗效的有效指标。Objective To evaluate the efficacy of ＂YangYin Runfei prescription＂ in the treatment of small cell lung cancer u- sing two different evaluation methods of traditional Chinese medicine and Western medicine combined with serum tumor markers of small cell lung cancer Pro - GRP and NSE. Methods Seventy - one cases of small cell lung cancer, including 30 limited cases and 41 advanced cases, were collected and divided into chemotherapy group（ 33 cases） and combined therapy group（ 38 cases）. The curative effect of the 71 cases was evaluated on clinical symptoms, survival quality, toxicity reaction, tumor cura- tive effect and serum markers Pro - GRP and NSE according to Clinical benefit evaluation criteria of advanced lung cancer by Chinese medicine treatment, WHO classification standard for anticaneer drug toxicity and Response evaluation criteria in solid tumors（RECIST）. Results According to Response evaluation criteria in solid tumors（RECIST）, the therapeutic effect had no statistical significance between two groups （ P 〉 0.05 ）. According to Clinical benefit evaluation criteria of advanced lung cancer by Chinese medicine treatment and WHO classification standard for anticancer drug toxicity, the combined group was superior to the chemotherapy group in improving clinical symptoms, survival quality and reducing toxicity reaction （ P 〈 0.05 ）. After 6 courses of chemotherapy, the decline of serum markers Pro - GRP and NSE in combined group was more obvious than chemo- therapy group （P 〈 0.05）. Conclusion ＂YangYin Runfei prescription＂ has attenuated synergistic effect in the treatment of small cell lung cancer. Serum tumor markers Pro - GRP and NSE are effective indicators in monitoring small cell lung cancer.
