摘  要:目的:探讨冷冻快速切片的免疫组化技术(IHC)辅助诊断乳腺癌前哨淋巴结转移灶的意义。方法:所有患者在术中均进行原发肿瘤和前哨淋巴结的切除,进行术中快速冷冻切片诊断(IRFS),对所有前哨淋巴结进行免疫组织化学检测。结果:单独术中冷冻切片诊断的阳性检出率为26%(13/50),术中快速冷冻切片结合免疫组化检测的阳性检出率为48%(24/50),两种方法的检测结果差异有统计学意义(P=0.001);IRFS结合IHC诊断乳腺癌前哨淋巴结转移癌的敏感性(100%)显著高于单独IRFS的敏感性(54.17%),但特异性没有差别。结论:快速冷冻切片的免疫组化检测在乳腺癌前哨淋巴结转移癌的诊断中具有重要应用价值。Objective: To discuss the auxiliary diagnosis by applying immunohistochemical (IHC) technology of rapid intraoperative frozen section (IRFS) to detect the sentinel lymph node metastases in patients with breast cancer. Methods: All patients were diagnosed for primary tumor and sentinel lymph node by IRFS, and all the sentinel lymph nodes were assayed by IHC method. Resuits: The positive detection rate of IRFS diagnosis alone was 26% (13/50), the positive detection rate of IRFS combined with IHC assay was 48% (24/50). There was difference of the detection results by the two different methods (P=0. 001), indicating the positive detection rate of the IRFScombined with IHC assay was higher The result showed that the sensitivity of IRFS combined with IHC in the diagnosis of the sentinel lymph node metastasis of breast cancer was 100%, significantly higher than that of IRFS alone (54.17%), but there was no difference in specificity between them. Conclusion. IHC has important application value in the detection of sentinel lymph node metastasis of breast carcinoma.
