听觉认知P300电位在评价精神分裂症患者认知损害中的价值研究

摘　　要：目的:探讨P300电位在评价精神分裂症患者认知损害中的价值。方法:将符合ICD-10精神分裂症诊断标准的56例患者作为试验组,正常健康人56例为对照组,于治疗前、治疗第4、12周时,进行P300测试、CPT及WCST评定。比较不同时期精神分裂症患者的P300电位和认知量表得分的变化。结果:与正常对照组比较,试验组各时期的P300潜伏期延长、波幅降低(P<0.05);试验组治疗12周后较治疗前P300潜伏期缩短,波幅增大(P<0.05)。试验组各阶段的WCST总测验次数、持续错误数、随机错误数均高于对照组(P<0.05),CPT分数低于对照组(P<0.05);治疗12周时,WCST的总测验次数、持续错误数、随机错误数均较治疗前降低(P<0.05),CPT分数较治疗前升高(P<0.05)。结论:精神分裂症患者存在不同程度认知损害,治疗对认知功能有改善作用,P300能反映精神分裂症患者的认知功能和预后状况。Objective： To explore the cognitive potentials of P300 evaluating cognitive impairment for schizophrenia patients. Method： 112 cases of schizophrenia were divided into two groups. The experiment group was in accord with ICD-10 Schizophrenia diagnostic criteria, and the control group was healthy. The P300 recording before the treatment and after treatment for 4 - 12 weeks were finished, and CPT, WCST questionnaires were assessed. The amplitude and the latencies of P300, the scores of the questionnaire were compared. Result： Compared with the control group, the experiment group＇s latencies of P300 were prolong, and the wave amplitude was lower （P〈0.05） . Compared with the prior treatment, the experiment group＇s latencies of P300 were shorten, and the amplitude was higher after 12 weeks＇ treatment （ P〈0.05 ） . Total trials, preservative errors, number of random errors were higher than the control group, and the CPT scores were lower than the control group. Compared with the prior treatment, the experiment group＇s total trials, preservative errors, number of random errors were lower, and the CPT scores were higher （ P〈0.05 ） . Conclusion： There were some cognitive impairment in the schizophrenia patients, treatment could improve the cognitive function, P300 can reflect the status of the cognitive function and prognosis of the patients with schizophrenia.
