摘 要:目的研究种植体支抗后牵引矫形力作用下上颌骨相关骨缝改建特点,探讨后牵引抑制上颌骨生长的作用机制。方法选用青春生长发育期的雄性恒河猴4只,随机分为3组(1.5月组1只、3月组2只和对照组1只)。实验组在其颧骨体部植入微钛板种植体,上颌佩戴全牙弓夹板,以单侧150g力的Ni-Ti螺旋拉簧连接上述部分,方向与平面平行,建立了颧骨种植体支抗后牵引上颌骨动物模型。采用组织学苏木精-伊红染色及计算机组织形态测定技术,定量分析骨缝反应的变化情况。结果骨缝宽度:在后牵引力作用下,颧颌缝、翼腭缝、横腭缝和颧颞缝骨缝宽度减小;腭中缝、额颌缝、蝶颧缝和颧额缝骨缝宽度增大。其中,额颌缝、蝶颧缝3月组增加较为明显,1.5月组与对照组相比差异无统计学意义;而颧颞缝、翼腭缝1.5月组减小较为明显,但与3月组比较差异无统计学意义。细胞密度:在后牵引力作用下,1.5月组各骨缝细胞密度均增大,随着牵引时间延长,颧颌缝、翼腭缝、横腭缝和颧颞缝细胞密度减小,呈现出压缩效应;腭中缝、额颌缝、蝶颧缝和颧额缝细胞密度增大,呈现出牵张效应,结果与骨缝宽度变化一致。但是随着牵引时间的进一步延长,骨缝的细胞密度变化略有差异。颧颌缝、翼腭缝、腭中缝和额颌缝1.5月组与3月组相比差异无统计学意义。结论后牵引力作用下,上颌骨有向后压缩并伴顺时针旋转趋势,颧骨作为支抗承载骨也有一定的旋转趋势。与正中矢状面有一定夹角的后牵引力对上颌腭中缝有横向扩弓的效应。Objective To examine the features of suture remodeling in the maxilla of rhesus monkey under backward trac- tive force, and to further explore the orthopedic mechanisms of the growth inhabitation of the maxilla subjected to the backward traction. Methods Four male adolescent rhesuses with mixed dentition were chosen and randomly divided into 3 groups(one an imal in the 1.5-month experimental group and control group,respectively;two in the 3 month experimental group and one in the control group). In the experimental groups, two titanium mini-plates were surgically fixed on the bilateral zygomatic bones as anchorages,and a backward force of 150 g was exerted on each side of the maxilla with a direction which was parallel to the oc- clusal plane. HE staining and computer assisted histomorphometry were used to quantitatively analyze the adaptive change of sutures responses. Results .After treatment with back ward tractive force, the width of the zygomaticomaxillary suture, trans- verse palatine suture,zygomatic temporal suture and ptery gopalatine suture became narrower whereas that of palatal suture,zy- gomaticofrontal suture,frontomaxillary suture and sphenomalar suture became broader. The width changes of the frontomaxil- lary suture and sphenomalar suture were more significant in the 3-month group. However, no significant difference in the width of the frontomaxillary and sphenomalar suture was found between the control group and the 1.5-month group. The width of zy- gomatic temporal suture and pterygopalatine suture was significantly decreased in the 1.5-month group with no significant difference found between the 1.5-month and 3-month groups. After backward tractive forcing,the cell density of all sutures was increased in the 1.5 month group. The cell density of the zygomaticomaxillary suture, transverse palatine suture, zygomatic tem- poral suture and pterygopalatine suture was decreased with time, suggesting a compressive effect, while that of the palatal su- ture, zygomaticofrontal suture, frontomaxillar
