摘 要:目的 探讨用流式细胞术检测大肠癌淋巴结微转移的方法学及意义。方法 首先将大肠癌Lovo细胞以不同浓度加入到非大肠癌患者的淋巴结组织制备的细胞悬液中 ,经异硫氰酸荧光素(FITC)标记的细胞角蛋白 (Cytokeratin ,AE1/AE3)和碘化丙锭 (PI)双重染色后 ,用盲法进行流式细胞术检测 ,建立参照模型。然后以同样方法测定 2 5例大肠癌患者经常规病理检查转移阴性的 16 2个淋巴结。结果 参照模型对Lovo细胞的检出值与理论值呈较理想的一致性 (r=0 976 ) ,可以从 10 5个淋巴细胞中检出 10个癌细胞。在 7例 (2 8% )大肠癌患者的 38个 (2 3 45 % )常规切片诊断阴性的淋巴结中检到了癌细胞 ,并与癌大小和浸润深度有相关性。结论 流式细胞术检测大肠癌淋巴结微转移是一种可靠、快速及定量的理想方法。Objective To study the methodology and significance of flow cytometry in detecting lymph node micrometastasis of colorectal cancer. Methods One hundred sixty two cellular suspensions were prepared with lymph nodes which were resected radically on 25 patients with colorectal cancer and in which no cancer cells were found by HE staining. Different concentrations of cultured Lovo colorectal cancer cells were added into the celular suspension prepared from lymph node tissue of persons without colorectal cancer in order to prepare a control model. Dual staining with CK/FTTC and PI was made to the sedimetns from those 2 kinds of suspension. Flow cytometry was used to detect cancer cells. Results An ideal correlation was obtained between the detection value and the theoretical value of cancer cells in the specimen suspensions and control models ( r = 0 097 6) with a sensitivity rate of 10/10 5. Cancer cells were detected from 7 out of the 25 patients and 30 of the 162 cellular suspensions. The detection rate was correlated with the size and infiltrating depth of the cancer. Conclusion Flow cytometry is a reliable, rapid, and quantitative method for detcting lymph node micrometastasis in colorectal cancer.
