喉癌患者外周血细胞免疫水平与预后的关系

摘　　要：目的：探讨喉癌患者外周血细胞免疫水平与预后的关系。方法采用流式细胞术检测41例喉癌患者（其中Ⅰ～Ⅱ期17例，Ⅲ～Ⅳ期24例；无淋巴结转移28例，有淋巴结转移13例；术后复发转移11例，无复发转移30例）手术前后外周血淋巴细胞（ CD3＋、CD4＋、CD8＋、CD19＋）及自然杀伤细胞（ NK）水平。结果手术后41例患者NK水平升高， CD19＋细胞水平下降（ P＜0．05）， CD3＋、CD4＋、CD8＋水平手术前后比较差异无统计学意义（ P＞0．05）；与Ⅲ～Ⅳ期患者比较，Ⅰ～Ⅱ期患者血CD4＋细胞水平显著降低，CD19＋细胞水平显著升高（P＜0．05）；有淋巴结转移喉癌患者外周血CD4＋细胞水平显著下降，NK细胞水平显著升高（P＜0．05）；术后随访1年，复发或转移组外周血CD4＋细胞水平显著下降，NK、CD19＋细胞水平显著升高（P＜0．05）。结论喉癌患者外周血细胞免疫水平对转移及预后判断有一定的临床参考价值。Objective To explore the relationship between the immunity level of peripheral blood cells and prognosis in patients with laryngeal cancer .Methods Flow cytometry was used to detecte the levels of peripheral blood lymphocytes（CD3 ＋,CD4 ＋,CD8 ＋,CD19 ＋） and natural killer（NK） cells before and after surgery in 41 cases of laryngeal cancer （17 of stage Ⅰ-Ⅱ,24 of stage Ⅲ-Ⅳ;28 without lymph node metastasis ,13 with lymph node metastasis;11 with recurrence metastasis ,30 without recurrence metastasis ） .Results The level of NK cell increased and the level of CD19＋cell decreased in 41 patients after surgery （P〈0.05）,the levels of CD3＋,CD4＋,CD8＋before and after surgery showed no statistical significance （P〉0.05）.The level of CD4 ＋cells in peripheral blood of patients with stageⅠ-Ⅱwas significantly lower in contrast with that of patients with stage Ⅲ-Ⅳwhile the level of CD 19＋cell increased significantly（P〈0.05）.In laryngeal cancer patients with lymph node metastasis ,the level of CD4 ＋cell decreased significantly while the level of NK cell increased significantly （P〈0.05）.After 1-year postoperative follow-up, the level of CD4＋cell decreased significantly while the levels of NK cell ,CD19＋cell increased significantly in recurrent or metastatic patients（P〈0.05）.Conclusion The immunity level of peripheral blood cells is of some clinical references for diagnosing the metastasis and prognosis of laryngeal cancer .
