摘 要:目的比较光相干生物测量仪(IOLMaster)和超声生物测量仪(接触式A型超声)对白内障合并视网膜脱离(累及黄斑区)患者眼轴长度(axialLength,AL)的测量结果,得出更加准确的AL,以进一步提高这类患者术后的视力。方法临床病例对照研究。对2009年5月至2012年10月在深圳市眼科医院就诊的35例35只眼(均为视网膜脱离累及到黄斑区的患者)分别用IOLMaster和接触式A超进行10次连续测量AL,并与B超粗测的AL进行对比。结果IOLMaster和接触式A超、B超测量AL(均值)分别为:(21.42+0.41)mm、(25.71±0.45)mm、(25.97±0.65)mm。IOLMaster和接触式A超测量的AL差异有统计学意义(t=-3.521;P=0.015)。接触式A超和B超测量的AL差异无统计学意义(t=-1.213;P=0-313)。结论对于白内障合并视网膜脱离(累及黄斑区)眼,IOLMaster测量的AL的可重复性和准确性均不再优于接触式A超。现阶段两种检查方法要针对患者的情况相结合使用,并联合B超以加强测量的精确度。Objective To compare the axial length (AL) measured by IOL Master and contact A-Scan in patients with cataract combined with retina detachment (RD) implicated with macula lutea, to obtain the accurate measurements of axial length to improve the vision of cataract patients. Methods Thirty-five eyes (35 patients) with cataract combined with RD were enrolled in this study. AL was measured with IOL Master and contact A-Scan. And the measurements were compared with that by b-mode ultrasound. Results Mean ALS by IOL Master and contact A-Scan and b-mode ultrasound were (20.42±0.41)mm, (25.16±0.45)mm, (24.97±0.65)mm. The difference between IOL Master and contact A-Scan was meaningful statistically (t =-3.521; P =0.015). Conclusions The results showed that measurements for AL are no longer precise and high reproducibility with IOL Master.
