摘 要:目的探讨高眼压性青光眼(high-tension glaucoma,HTG)和正常眼压性青光眼(normal-tension glaucoma,NTG)图形视觉诱发电位(pattern visual evoked potentials,P-VEP)与视野缺损的相关性。方法选取青光眼患者89例(89眼),其中NTG患者58例,HTG患者31例,对两组患者的一般情况(年龄、性别、屈光度、瞳孔直径等)进行比较分析,所有患者进行常规视野检查,检查指标包括图形标准偏差(pattern standard deviation,PSD)和平均视野缺损(mean defect,MD),另外所有患者行P-VEP检查,观察P100波潜伏期与振幅。同时,对P-VEP的P100波潜伏期和振幅与视野缺损的相关性进行统计分析。结果 NTG组患者的角膜厚度和眼压明显低于HTG组,差异均有统计学意义(均为P<0.01);NTG和HTG组患者的P100波潜伏期分别为(115.46±10.83)ms、(116.62±11.47)ms,振幅分别为(9.15±4.36)μV、(8.31±2.94)μV,差异均无统计学意义(均为P>0.05);晚期NTG患者的屈光度明显低于HTG组,差异有统计学意义(P=0.04);P-VEP检查的P100波潜伏期与常规视野检查的NTG和HTG患者MD值明显负相关(均为P<0.05),而与PSD值则无明显的相关性(均为P>0.05)。结论各期HTG和NTG患者间的P100波潜伏期和振幅无明显差异,而视野检查的MD值与P-VEP检查的P100波潜伏期呈负相关。Objective To investigate the correlation between pattern visual e- voked potentials(P-VEP) parameters and visual feld defect in normal-tension glaucoma (NTG) and hypertension glaucoma(HTG). Methods A total of 89 glaucoma patients (89 eyes) ,including 58 cases with NTG and 31 cases with HTG,were chosen,the gener- al information of patients ( sexual, age, diopter, pupil diameter, et al ) between two groups were compared, the routine visual filed examination was performed, the indexes included pattern standard deviation( PSD), and mean defect (MD), and the latency time and amplitude of P100 wave in P-VEP were recorded. The correlations between P-VEP parameters and visual field defect of the NTG and HTG groups were evaluated. Results The corneal thickness and the intraocular pressure in NTG group were lower than those in HTG group, there were statistical differences ( all P 〈 0.01 ). The latency time and amplitude of P100 wave in P-VEP in NTG group were( 115.45 ± 10.83)ms and(9.15 ±4.35) μV,respectively,which in HTG group were ( 116.52 ±11. 47 ) ms and ( 8.31 ± 2.94 )μV, there was no statistical difference between two groups ( all P 〉 0.05 ). In advanced NTG patients, the diopter were lower than HTG patients, there was statistical difference (P = 0.04 ). The latency time of P100 wave in VEP was significantly negative correlated with MD value in NTG and HTG patients( all P 〈 0. 05 ), but not correlated with PSD ( all P 〉 0.05 ). Conclusion There is no statistical difference between NTG and HTG patients in latency time and amplitude of P100 wave in P-VEP. The latency time of P100 wave in VEP is significantly negative correlated with MD value.
