摘 要:目的探讨抗青光眼术后中央角膜厚度(central corneal thickness,CCT)变化及其与眼压(intraocular pressure,IOP)的相关性。方法选取2009年10月至2011年12月于我院就诊的青光眼患者40例(60眼),采用穿透性小梁切除术结合周边虹膜切除术或非穿透性小梁手术,分别于术前及术后1周、1个月、3个月、6个月行常规检查,包括CCT、IOP、裂隙灯检查等。采用Spearman法分析CCT与IOP之间的相关性。结果术后1周CCT(539.8±44.8)μm,与术前(556.6±42.6)μm相比,差异有统计学意义(P〈0.05);1个月后CCT略有上升,3个月与6个月后CCT变化甚微,术后不同时间点间CCT差异均无统计学意义(均为P〉0.05)。术后1周IOP显著降低,与术前相比,差异有统计学意义(P〈0.05);术后1个月稍微升高,术后3~6个月变化不大,较为稳定,术后不同时间点间IOP差异均无统计学意义(均为P〉0.05)。术前CCT与IOP无相关性(r=0.110,P=0.910),虽然术后1周CCT与IOP呈一定程度的正相关性(r=0.317,P=0.045),但相关性较低,术后1个月、3个月、6个月CCT与IOP均无相关性(r=0.196、0.101、0.051,P=0.312、0.453、0.702),因此可以认为手术前后IOP与CCT值均无明显相关性。结论抗青光眼术后CCT下降.其与IOP相关性不明显。Objective To discuss the changes of central corneal thickness(CCT) after glaucoma surgery, and analyze its relationship with intraocular pressure (IOP). Methods A total of 60 eyes in 40 patients with glaucoma were chosen,all patients re- ceived the trabeculectomy combined with periphery iridectomy or non-penetrating trab- eculectomy, the CCT, IOP at pre-operation and postoperative 1 week, 1 month, 3 months,6 months were checked,the correlation between CCT and IOP were analyzed by Spearman methods. Results CCT at postoperative 1 week was ( 539. 8 ± 44. 8 ) μm,compared with pre-operation ( 556.6 ± 42.6 ) μm, there was statistical difference (P 〈 0.05 ). CCT at postoperative 1 month slightly increased, stabled at postoperative 3 months and 6 months, there was no statistical difference between postoperative time points( all P 〉 0. 05 ). IOP at postoperative 1 week was the lowest, compared with pre- operation,there was statistical difference (P 〈 0. 05 ). IOP at postoperative 1 month slightly increased, stabled at postoperative 3 months and 6 months, there was no statisti- cal difference between postoperative time points(all P 〉0.05). There was no correlation between preoperative CCT and IOP(r =0. ll0,P =0. 910) ,they correlated at postoperative 1 week( r =0. 317 ,P =0. 045 ) ,but the coefficient was very low,there was no statistical difference at postoperative 1 month,3 months and 5 months between CCT and IOP( r = 0. 195,0. 101,0. 051 ,P =0. 312,0. 453,0. 702), so it could be considered that there was no correlation between CCT and IOP. Coneluuion CCT decrease after glaucoma surgery and it has no obvious correlation with IOP.
