摘  要:目的:测量无牙颌患者总义齿修复前后软组织三维面像的改变,为总义齿修复结果预测提供数据支持。方法:选取20例AtwoodⅡ类牙槽嵴吸收的无牙颌患者,使用DSC-2型面部三维成像系统进行总义齿修复术前、术后面部轮廓数据获取和重建,并将每一患者术前、术后三维面像进行重叠,建立坐标系,评价术前、术后软组织三维轮廓的改变。结果:AtwoodⅡ类牙槽嵴吸收的患者总义齿修复后面像改变主要集中在上唇及口角部位,下唇及颏部位移不明显,前后方向上的位移>垂直方向上的位移>左右方向上的位移。总义齿修复术后面部关键距离变化,除左-右口角间距稍有减小外,左-右上唇缘点间距、左-右下唇缘点间距、上-下唇凹点间距、上-下唇凸点间距均增加。结论:通过三维面部照相及测量分析,可以定量评价总义齿修复前后的颅颌面软组织三维特征变化,有助于总义齿修复结果预测,提高患者的满意程度。Objective: To collect and analyze the three-dimensional (3D) facial profile variation of edentulous patients restored by complete denture. Methods: The 3 D facial images of 20 Atwood Class I1 edentulous patients were taken by DSC-2 3D facial imaging system before and after restoration of complete denture. Then, the two 3D facial images of each patient before and after restoration were registered in the same coordinate system by partial structure overlap. The differences between these two 3D facial images were statistically analyzed. Results: The major facial profile variation of Atwood Class I1 edentulous pa- tients restored by complete denture focused in the area around the upper lip and the comer of the mouth. The change of lower lip and chin area was not obvious. The shift in the sagittal direction was significantly more evident than that in the vertical direction, while the shift in the horizontal direction was the least one. Conclusion: The pattern of facial profile variation of edentulous patients restored by complete den- ture based on 3 D imaging may provide reliable data support for prediction and communication in clinical practice.
