摘　　要：目的探讨头颈部肿瘤患者放疗期间营养状况的影响因素。方法运用SAS6.0的因子分析法得出放疗期间(放疗前、放疗第2周、第4周、放疗结束时)营养状况综合评分作为因变量,以患者人口学资料和临床特征资料作为自变量,采用广义估计方程分析患者营养状况的影响因素。结果年龄、居住地、体重、鼻咽癌、T分期、手术史、化疗史、放疗史、放疗方式和放疗次数是影响患者营养不良的主要因素。结论针对年老者、农民、入院前体重下降大于5%、鼻咽癌、未行手术、有放化疗史且放疗次数多等因素存在时,应考虑在放疗前给予营养支持,改善患者营养状况,减轻放疗不良反应,提高患者的生活质量。Objective The study aimed to explore the influencing factors for nutritional status of patients with head and neck cancer during radiotherapy. Methods Overall scores on nutritional status during radiotherapy （before therapy, 2 weeks into therapy, 4 weeks into therapy, and end of therapy） as determined by factor analysis in SAS6.0 were regarded as the dependent variables, and patients＇ demographical information and clinical features were regarded as the independent variables; generalized estimating equa- tion was adopted to analyze the influencing factors for nutritional status of the patients. Results Age, place of residence, weight, na- sopharynx cancer, T-stage, surgical history, chemotherapy record, radiotherapy record, types of radiotherapy, and times of radio- therapy were the main factors for malnutrition of the patients. Conclusion For patients who are elderly or farmers, have lost weight for more than 5% prior to hospitalization, have nasopharynx cancer, have not received a surgery, or have received a chemotherapy with multiple radiotherapies, nutritional support should be provided prior to starting a radiotherapy, so to improve nutritional status of the patients, reduce adverse reactions caused by radiotherapies, and enhance the quality of life of the patients.
