透明角膜切口超声乳化术后稳定性研究

摘　　要：目的通过眼前段光学相干扫捕仪（Opticalcoherencetomography，OCT）观察白内障超声乳化吸除术后透明角膜切口24h内的形态变化，探讨切口的安全性和稳定性。方法临床病例系列研究。对2011年5月至2012年3月在广州爱尔眼科医院经透明角膜切口进行超声乳化向内障吸除术的老年性自内障患者50例（50只眼），术后第5、12、24h应用眼前段OCT观察对切口的形态学变化；非接触眼压计测量眼压变化；裂隙灯显微镜进行常规检查。结果在术后第5h切口的眼压值最低差异有统计学意义（P〈0．05）；其切口发生形态学变化OCT观察外切口哆开、内切口哆开、后弹力层脱离等发生率均高于其他2个时间点差异有统计学意义（P〈0．05），故术后5h是该术式关键期。结论透明角膜切口超声乳化自内障吸除术切口的早期闭合，尤其是术后5h内的闭合是实现透明角膜切口安全．眭恢复，防止眼内感染的关键时期。Objective To evaluate the effect in morphology and intraocular pressure （IOP） in- duced by clear corneal incisions after phacoemulsification. Methods Optical coherence tomography （OCT） and tonometry were performed on 50 eyes at 5, 12, and 24 hours after cataract surgery. Results At 5 hours after surgery, patients with incisions had lower IOPs as measured by noncontact tonometry compared with the IOPs at the other two time points （P 〈0.05）. The incidences of endo- thelial and epithelium gap at 5 hours after surgery was significantly higher at 12 and 24 hours （P 〈 0.05）. The incidence of descemet membrane detachment at 5 hours was also significantly higher than in it at the other two points （P 〈0.05）. Conclusions For these patients, eyes may be especial- ly vulnerable within 5 hours of cataract surgery.
