前房型人工晶状体植入对角膜内皮影响观察

摘　　要：目的探讨无晶状体眼前房型人工晶状体植入对角膜内皮的影响。方法回顾性临床病例自身对照研究。对2008年1月至2010年12月在大连市第三人民医院眼科就诊患者选取晶状体脱位、晶状体囊内摘除19例19只眼，术眼为观察组，对侧眼为对照组，术前进行最佳矫正视力、眼内压、角膜内皮镜检查。分别于治疗后1周、4周、6个月、12个月、24个月进行随诊，记录最佳矫正视力、眼内压和角膜内皮镜检查结果及并发症等情况。结果前房型人工晶状体植入组术后1周、4周、6个月、12个月、24个月的角膜内皮细胞密度及角膜内皮细胞形态与对照组相比差异无统计学意义（P〉0．05），前房型人工晶状体植入组术前和术后最佳矫正视力比较有明显提高，差异具有统计学意义（P〈0．05），术后随访各时间段视力未见明显变化（P〉0．05），观察组眼内压术前术后没有明显变化（P〉0．05），未见其它与前房型人工晶状体植入有关的眼部不良反应。结论无晶状体眼新型弹性开放襻前房型人工晶状体植入是安全有效的，可以有效提高患者视力，改善患者生活质量，其相对操作简便、对组织损伤小、经济实用、患者痛苦小，是治疗无晶状体眼人工晶状体植入的一种有效方法。Objective To investigate the endothelial changes after implantation of anterior cham- ber intraoeular lens （AC IOL） in cataract surgery. Methods This retrospective study comprised 19 eyes of 19 patients who had implantation of an AC IOL. The specular microscope was used to per- form full morphometric analysis of the corneal endothelium. The values obtained were compared with those in fellow eyes, which were phakic. Results The endothelial cell density and endothelial morphometry after 1 week, 4 weeks, 6 months, 12 and 24 months compared with those control pha- kic eyes were not significantly different （P 〉0.1） in statistics. The visual acuity was obviously im- proved after operation （P 〈0.05）, and the BCVA difference of 1 week, 4 weeks, 6 months, 12 and 24 months were not significant （P 〉0.1） in statistics. The post operation intraocular pressure had no significant difference （P 〉0.1） from the values before operation. No other ocular adverse events were found during the follow-up duration. Conclusions Implantation of AC IOL is safe and effective for aphakic patients with good vision acuity after operation by simply operation and less suffering in the short-term.
