口腔颌面部间隙感染201例临床资料分析

摘　　要：目的探讨颌面部间隙感染的病因、治疗方法与处理要点。分析年龄与全身状况对颌面部间隙感染的影响。方法对201例颌面部间隙感染病例,从病因、年龄、全身状况、致病菌、诊断、治疗等方面进行回顾性研究。结果 201例患者中,感染源以牙源性者最多,有154例(76.6%);腺源性次之,有41例(20.4%)。经综合治疗,189例(94%)痊愈出院,12例出现并发症,经积极抢救和治疗,9例痊愈,3例死亡,死亡者均为60岁以上伴有全身慢性疾病老年患者。结论牙原性感染是颌面部间隙感染的首要病因。有基础性疾病的老年患者容易出现严重并发症。对颌面部间隙感染需及时诊断,应用足量敏感抗生素、脓肿形成后及时切开引流、积极控制全身基础性疾病等综合治疗措施可提高颌面部间隙感染治愈率,减少并发症。Objective To discuss the cause and treatment of oral and maxillofacial space infections （ MSI）, and the effects of ages and the states of systemic factors in tatment. Methods A retrospective review was conducted on the clinical data of 201 patients who were diagnosed as having MSI, including the cause of diseases, ages, associated systemic diseases, pathogenicbacteria, treatments, the points of treatments, effects and so on. Results of the 201 patients, 154 patients （76.6% ） were due to the odontogenic infections, fnllowed by 41 patients （20.4%） resulted from the infec tion from glands. 189 patients （94%） got well while 12 emerged complication. 9 out of them got well through treat ments. And another 3 died and they were over 60 years old and had chronic diseases. Conclusion The odontogenic in- fectiuns is the main reason of the oral and maxillotacial space infections. The old patients who have underlying diseases will appear complication easily. It is crucial to make an exact diagnosis about oral and maxillofacial space infections in time. Llse enough susceptible antibiotics, incision and drainage ＇after the abscess is taking shape, control associated systemic diseases and so on to treat oral and maxillofacial space infections. That can improve recoveryrate and reduce the oc-currence of complication.
