组织多肽特异性抗原联合癌抗原125检测在口腔癌诊断中的临床价值

摘　　要：目的探讨口腔癌患者血清中组织多肽特异性抗原(TPS)和癌抗原125(CA125)水平对口腔癌的临床诊断及病情监控的临床意义。方法用ELISA方法检测46例口腔癌和46例口腔良性肿瘤患者及46例健康对照者血清及唾液中TPS水平,同时用电化学发光法检测血清及唾液CA125水平。结果口腔癌组的TPS、CA125水平明显高于口腔良性肿瘤组和健康对照组(P<0.05);进展期口腔癌患者的血清及唾液中TPS和CA125明显高于稳定期患者(P<0.05)。治疗后不同预后患者的TPS和CA125浓度差异有统计学意义(P<0.05)。TPS联合CA125检测敏感性及特异性均高于TPS、CA125单独检测的敏感性及特异性。结论血清TPS联合CA125检测可作为口腔癌早期诊断的指标,对疾病的诊断及预后具有很高的临床价值。Objective To explore the clinical values of tissue polypetide specific antigen（TPS） and CA125 in sera of patients with oral carcinoma and their significance in the diagnosis and the disease monitoring.Methods Serum and saliva levels of TPS and CA125 were detected using ELISA and ECL in 46 patients with oral carcinoma,46 patients with benign oral tumors and 46 healthy controls,respectively.Results The serum and saliva levels of TPS and CA125 in oral cancer group were significantly higher than those in benign oral tumor group and healthy group（P ＜ 0.01）.The serum levels of TPS and CA125 in patients with oral cancer at progressive stage were significantly higher than those in patients at stationary stage（P ＜0.05）.The levels of TPS and CA125 showed statistically significant difference among oral carcinoma patients with different prognosis （P ＜ 0.05）.Combined determination of TPS and CA125 showed the higher sensitivity and specificity than that of TPS or CA125 alone,respectively.Conclusion Serums and saliva levels of TPS and CA125 are useful in diagnosis of oral cancer and monitoring of the disease progression.
