摘 要:目的比较超高度近视患者戴用框架镜和硬性高透氧角膜接触镜(RGPCL)后的视力差异及影响因素。方法对62例(113只眼)超高度近视患者验配RGPCL,观察验配RGPCL后的最佳矫正视力(BCVA)及配适情况,定期复诊,观察有无眼部并发症,并与框架镜配戴进行比较。结果 113只眼配戴RGPCL后BCVA 4.89±0.16,配戴框架眼镜后BCVA 4.70±0.27。配戴RGPCL最佳矫正视力均好于或等于配戴框架眼镜的最佳矫正视力,平均视力提高均值为0.19±0.16,二者比较有显著统计学差异(t=38.71,P<0.01)。统计显示近视度数越高者戴用RGPCL比戴用框架眼镜视力提高越明显。最佳矫正视力的结果与等效球镜屈光度和验配RGPCL前是否进行屈光矫正等因素密切相关。结论配戴RGPCL可减少框架镜带来的像差、物像畸变及视野影响等不足,使患者获得良好的视觉质量,有较好的舒适度,为超高度近视患者视力矫正的理想选择。Objective To compare the visual acuity changes and related factors in high myopia patients wearing rigid gas permeable contact lens ( RGPCLs ) versus wearing spectacles. Methods Totally 62 extremely high myopic pa- tients ( 113 eyes) were examined and fitted with RGPCLs. Best corrected visual acuity ( BCVA) and compatibility were ob- served and compared with these in subjects who wear spectacles. Regular follow-up were maintained and ocular complica- tions were observed. Results The BCVAs of the eyes wearing RGPCLs and wearing spectacles were 4.89 ±0. 16 and 4.70 ± 0.27 respectively. BCVA in patients wearing RGPCLs were superior or equal to those wearing spectacles, with an aver- age improvement of 0.19±0. 16 ( t = 38.71, P 〈 0.01 ). Statistics indicated that the higher the myopia was, the more significant improvement the patients were experienced. The BCVAs were closely related to spherical equivalent and refrac- tive correction before fitting with RGPCLs. Conclusion RGPCLs are the ideal choice for super high myopic patients for vi- sion correction. RGPCLs provide improved visual quality and comfort by reduced optical aberrations and distortions and wi- der visual field.
