摘 要:目的 探讨高迁移率族蛋白A1(HMGAl)表达与胸中段食管鳞癌患者预后的关系.方法 收集手术治疗189例胸中段食管癌患者临床资料,采用免疫组织化学法进行HMGA1检测,采用Kaplan-meier法进行生存分析、用Cox回归分析判定独立预后因素.结果 T1、T2和T3患者HMGA1表达率分别为54.5%、61.4%和78.5%(P<0.05);有、无淋巴结转移患者HMGA1表达率分别为82.7%和63.9% (P<0.01).T2患者中,有、无HMGA1蛋白表达者5年生存率分别为.34.3%和68.2% (P <0.05);T3患者中,有、无HMGA1蛋白表达组的5年生存率分别为25.3%和54.5% (P<0.05).无淋巴结转移(pN0)患者中,有、无HMGA1蛋白表达患者5年生存率分别为42.0%和66.7% (P <0.05);有淋巴结转移(pNl)患者中,有、无HMGA1蛋白表达患者5年生存率分别为14.9%和50.0% (P<0.05).结论 食管鳞癌不同的T、N分期中HMGA1蛋白表达存在差异;HMGA1蛋白过度表达者5年生存率降低;肿瘤浸润、淋巴结转移和HMGA1蛋白过度表达是独立的不利预后因素.Objective To investigate the expression of high mobility group A1 (HMGA1) protein and its relationship to the prognosis of esophageal cancer after esophagectomy.Methods One hundred and eighty-nine patients with squamous cell carcinoma at the middle-third of the esophagus were enrolled in this study.The expression of HMGA1 protein was detected by using immunohistochemistry.According to the clinicopathologic factors,the difference of HMGA1 protein expression was compared by x2 test.Kaplanmeier method was performed to calculate the survival rate.Cox regression multivariate analysis was performed to determine independent prognostic factors.Results The HMGA1 protein expression rate in T1 stage,T2 stage and T3 stage was separately 54.5%,61.4% and 78.5% (P < 0.05).The positive expression rate of HMGA1 protein in patients with lymph node metastasis was significantly higher than in those without metastasis (P < 0.01).The 5-year survival rate in patients positive for the expression of HMGA1 protein was significantly lower than in those negative for the expression of HMGA1 protein (P <0.01),that in T2 and T3 stage patients positive for the expression of HMGA1 protein was significantly lower than in those negative for the HMGA1 protein expression (P < 0.05),that in N0 stage patients positive for the HMGA1 protein expression was significantly lower than in negative for the HMGA1 protein expression (P < 0.05),and that in N1 stage patients positive for the HMGA1 protein expression was significantly lower than in those negative for the HMGA1 protein expression (P < 0.05).The result of Cox analysis demonstrated that T stage,N stage and HMGA1 protein positive expression were independent prognostic factors.Conclusion HMGA1 protein overexpression was detected in esophageal squamous cell carcinoma and was found to be significantly associated with T stage and lymph node metastasis.The 5-year survival rate in the patients with HMGA1 protein overexpression was significantly lower than in tho
