平时和战争背景下军人原发性头痛残疾程度评估

摘　　要：目的评估平时和战争背景下军人原发性头痛残疾程度,为开展原发性头痛的防治提供参考依据。方法选定北方战区的在外驻训某部队,驻训前对官兵进行体检,筛选出偏头痛、紧张性头痛、慢性偏头痛病例各40例和健康军人40例,根据偏头痛残疾程度评估问卷(migraine disability assessment,MIDAS),进行调查问卷,驻训3个月后再次进行调查问卷。对所有病例的残疾程度进行分级,然后比较各组之间的区别。结果平时与对照组相比,偏头痛组总分及各项得分偏高(P<0.05)。不同学历偏头痛及紧张型头痛患者之间得分无显著性差异(P>0.05)。在战争背景下,各组得分明显升高(P<0.05),慢性偏头痛组上升更加显著。偏头痛组评分多为Ⅲ级(30.0%),慢性偏头痛组评分多为Ⅳ级(32.5%)。结论与平时相比,在战争背景下各类原发性头痛的致残程度均增加,其中慢性偏头痛具有更高的残疾级别。原发性头痛尤其是慢性偏头痛可影响战时部队战斗力。Objective To evaluate the disability degree of primary headache in military during peacetime and war- time, and provide reference for the prevention. Methods A certain troop training in the field in the northern command was selected for the study. The servicemen had a physical examination before the field training. The cases with migraine （40 cases）, tension headache （40 cases） and chronic migraine （40 cases） and 40 health servicemen were screened. The questionnaire survey was conducted using migraine disability assessment （MIDAS） then and three months after the field training. The disability degrees of all the cases were evaluated and compared among the different groups. Results Com- pared with the control group, the aggregate score and the item scores were higher ofthe migraine groups （P〈0. 05） dur- ing peacetime. There was no significant difference （P〈0. 05） of the score between the groups with migraine and tension headache with different education backgrounds. The scores of all the groups increased obviously during wartime （P〈0.05） . The group with chronic migraine had more significant increase in these scores. The migraine group was mostly e- valuated as grade III （30. 0%）, and the chronic migraine group was mostly evaluated as grade IV （32. 5%）. Conclusion Compared with the peacetime, the disability degrees of all kinds of primary headaches rise during wartime. Patients with chronic migraine have higher disability degree. The primary headache, especially the chronic migraine, can influence the battle effectiveness during wartime.
