生物羊膜在60岁以上老人原发性闭角青光眼滤过手术中应用的临床观察

摘　　要：目的探讨生物羊膜在60岁以上老人原发性闭角型青光眼滤过手术中应用效果。方法对临床收治的60岁以上原发性闭角型青光眼58例(70只眼)行小梁切除联合羊膜移植术观察其临床疗效,包括眼压,滤过泡,视力及并发症等方面的评估。结果术后第1天,第1周,第1、3、6月眼压均控制良手术成功率高,副反应小。讨论 60岁以上老人眼球尤其是角膜缘处发生特殊的组织学改变,使其青光眼滤过手术中丝裂霉素C等的使用更加受限,而生物羊膜副反应较小,且可以长期有效的降低眼压,提高手术成功率。Objetive To explore the application effect of biological amnion in the filtering operation of primary angle closure glaucoma patients older than 60 years. Methods 58 cases （70 eyes） of primary angle closure glaucoma older than 60 years underwent trabeculectomy with amniotic membrane transplantation. The clinical curative effect, intraocular pressure, filtering bleb, visual acuity and complications were evaluated. Results First day, 1 week, lmonth, 3 month and 6 month after the operation, all cases got good IOP. The curative rate is high and there are few complications. Conclusion Special tissue change in limbal of cases older than 60 years make the used of mitomycin C restricted. Bbiological amniotic membrane has fewer side effects, can effectively reduce the intraocular pressure for a long period of time and improve the success rate of operation.
