摘 要:目的探讨糖尿病足底压力异常患者行减压鞋垫治疗的效果及护理方法。方法使用比利时RS-SCAN 1米压力平板测定系统对357例2型糖尿病患者进行足底压力检测,筛选出41例严重足底压力异常患者。对严重足底压力异常患者采集足型,选取适合患者的鞋垫,进行打磨与修剪制作成型,指导患者穿鞋时放置减压鞋垫。分别于穿鞋垫3个月、6个月后再次分析足底压力,比较制作鞋垫前后足底压强及冲量的变化。结果 3个月后患者左、右足压强,6个月后右足压强显著低于穿鞋垫前(均P<0.05);3个月后双足冲量同穿鞋垫前比较差异无统计意义(均P>0.05),6个月后双足冲量同穿鞋垫前比较差异有统计学意义(均P<0.05)。结论糖尿病患者足底压力异常增高可通过减压鞋垫起到缓冲作用,随着患者穿鞋垫时间的延长,压力缓解作用会逐渐减弱,建议6个月后更换减压鞋垫。Objective To investigate the effect of decompression treatment on diabetic patients with abnormal plantar foot pressure. Methods A total of 357 type 2 diabetes patients were enrolled and asked to test their barefoot plantar pressure with a 1 m Fool Scan pressure plate (RSScan International, Belgium). Forty one of them were screened to have severe abnormal plantar foot pressure. For these patients, shape of feet was measured to model shoe pads. Then they were instructed to use the decompression shoe pads. Plantar foot pressure and impulse were measured at 3 and 6 months after starting the treatment. Results Subjects showed a signifi cant decrease in plantar pressure in both feet at 3 months after starting the treatment compared to the baseline level, whereas after 6 months only right foot pressure had a significant reduction(P〈0.05 for all). No significant differences were observed in impulse of both feet after 3 months(P〉0.05 for both), but significant differences were found after 6 months(P〈0.05 for both). Conclu- sion Decompression shoe pads create a cushion effect in diabetes patients with abnormal plantar foot pressure. Cushion effect could be weakened along with time goes by, so decompression shoe pads should be changed every 6 months.
