甲胎蛋白异质体在肝细胞癌早期诊断中的价值

摘　　要：目的探讨甲胎蛋白异质体（AFP—L3）在肝细胞癌（hepatocellular carcinoma，HCC）早期诊断中的价值。方法选择186例甲胎蛋白（α-Fetoprotein，AFP）水平为20～400ng／ml肝病患者的血清，应用微量凝集素亲和吸附离心管分离、洗脱获得AFP-L3，用化学发光免疫分析法检测AFP和AFP．L3含量，计算AFP—L3占血清AFP的百分比即AFP—L3百分数。结果HCC患者血清中AFP—L3水平及AFP—L3百分数明显高于慢性肝炎（chronichepatitis，cn）和肝硬化（1ivercirrhosis，LC）患者（P〈0．01）；以AFP—L3百分数≥10％为阳性，HCC患者AFP-L3的阳性率明显高于CH和LC患者。AFP-L3百分数诊断HCC的灵敏度是71．43％，特异度是92．36％。结论检测血清中AFP-L3水平对早期HCC的诊断有重要价值。[ Abstract] Objective To evaluate the clinical value of ct-Fetoprotein variants（AFP-L3） in early diagnosis of hepatocellular carcinoma. Methods The AFP-L3 was isolated from serum samples of 186 liver disease patients with serum AFP levels ranged from 20 to 400 ng/ml by using a micro-spin column equipped lens culinaris agglutinin（LCA）. The levels of AFP and AFP-L3 were determined by using an ARCHITECT i2000 System. The percentages of AFP-L3 （AFP-L3%）in AFP were calculated. Results The serum AFP-L3 levels and AFP-L3% in patients with early hepatoeellular carcinoma（HCC） were significantly higher than that in patients with chronic hepatitis （CH）or liver cirrhosis （LC） （P 〈 0. 01 ）. Taking AFP-L3%≥ 10% as diagnostic criteria, the positive rate in the patients with early HCC was significantly higher than that in the patients with CH or LC. The sensitivity of AFP-L3% in early HCC diagnosis was 71.43% and the specificity was 92. 36%. Conclusion The detection of AFP-L3 has great clinical value in early diagnosis of HCC.
