Fibrotouch与Fibroscan检测肝脏硬度的临床效能比较

摘　　要：目的评价国产第三代影像引导肝脏瞬时弹性检测仪(Fibrotouch,FT)与法国传统瞬时弹性成像仪(Fibroscan,FS)检测肝脏硬度的临床效能。方法乙肝肝纤维化90例、慢性乙型肝炎107例、健康志愿者37例,分别应用FT和FS两种仪器先后测定肝脏硬度弹性值,比较两种仪器检测的成功率,检测值差异,对肝纤维化诊断的敏感度、特异度及符合率。结果 FT检测成功率为100%,FS成功率为92.5%。乙肝肝纤维化组、慢性乙型肝炎组、健康志愿者组应用FS、FT方法检测肝脏硬度弹性均值比较,差异均无统计学意义[(24.38±10.96)kPa vs.(24.10±11.37)kPa,(8.58±3.78)kPa vs.(8.30±3.52)kPa,(5.00±1.48)kPa vs.(5.37±1.59)kPa;P均>0.05]。FT、FS对诊断肝纤维化的敏感度为83.3%vs.86.7%,特异度为97.2%vs.93.8%,符合率为91.9%vs.91.0%,但差异无统计学意义(P>0.05)。结论应用FT可有效进行无创肝脏硬度检测,与FS具有较好的一致性。Objective To compare the consistency of Fibrotouch and Fibroscan in the assessment of liver tissue elastic modulus. Methods Liver tissue elastic modulus in 90 patients with fibrosis,107 patients with chronic hepatitis B and 37 healthy volunteers were measured by Fibrotouch and Fibroscan respectively, then their consistency and clinical effectiveness were compared. Results The test success rate of Fibrotouch was 100.0%, and that of the Fibroscan was 92.5%. There was no significant difference of the liver tissue elastic modulus by Fibrotouch and Fibroscanin the hepatitis fibrosis[（24.38±10.96）kPa vs. （24.10±11.37）kPa, P 〉0.05], chronic hepatitis B[（8.58±3.78）kPa vs. （8.30±3.52）kPa, P 〉0.05] , healthy volunteers[（5.00±1.48）kPa vs. （5.37±1.59）kPa, P 〉0.05]. There was no significant difference in sensitivity （83.3% vs. 86.7%）, specificity（97.2% vs. 93.8%） and coincidence rate in the diagnosing of liver fibrosis（91.9% vs. 91.0%） by Fibrotouch and Fibroscan. Conclusion Both Fibrotouch and Fibroscan are non-invasive techniques and can effectively detect the consistency of liver tissue which could be used to reflect pathological changes and be helpful in the diagnosis of liver fibrosis.
