乳腺癌患者外周血淋巴细胞10种表型的检测及其意义

摘　　要：目的:通过比较乳腺癌患者与正常人群及女性肺癌患者外周血淋巴细胞10种表型的变化,探讨淋巴细胞亚群检测对乳腺癌患者的临床应用价值。方法:采用四色免疫荧光标记流式细胞仪分析技术。结果:与正常对照组比较,乳腺癌组CD3+、CD4+细胞百分数及CD4+/CD8+比值明显下降(P=0.000 0,P=0.036 1),而CD8+和CD3+CD16+CD56+细胞两组间比较差异无统计学意义(P=0.959 4,P=0.353 2),静止总T淋巴细胞比例下降(P=0.018 9)、活化B淋巴细胞和调节性T细胞比例升高(P=0.000 0,P=0.002 3);与女性肺癌组比较,乳腺癌组CD3+细胞和CD4+细胞明显下降(P=0.0021,P=0.000 2);CD8+细胞、CD3-CD16+CD56+细胞和CD4+/CD8+比值2组间比较差异无统计学意义(P=0.413 6,P=0.800 8,P=0.413 6),静止总T淋巴细胞和抑制性T细胞比例降低(P=0.010 2,P=0.021 7),活化总T淋巴细胞、活化B淋巴细胞、细胞毒性T细胞和调节性T细胞均升高(P=0.000 0,P=0.000 6,P=0.000 0,P=0.002 9)。结论:这些受检乳腺癌患者的免疫功能下降明显。但是可能仍处于代偿阶段,并未受到深度抑制。Objective： To explore the clinical application value of subsets detection of lymphocytes for patients with breast cancer by comparing the changes of ten phenotypes of peripheral blood lymphocytes in breast cancer patients and normal women, female patients with lung cancer. Methods： Four - color immunofluoresence - labeled flow cytometry was used in the study. Results： Compared with normal control group, the proportions of CD3^＋ cells and CD4^＋ cells, the ratio of CD4^＋/CD8^＋ decreased significantly （ P = 0. 000 0, P = 0. 036 1 ）, but there was no statistically significant difference in the proportions of CD8^＋ cells and CD3 ^＋ CD16 ^＋ CD56 ^＋ cells between the two groups （P =0. 959 4, P =0. 353 2） , the proportion of static total lymphocyte decreased （P =0. 018 9）, the proportions of active B lymphocytes and regulatory T lymphocytes increased （ P = 0. 000 0, P = 0. 002 3 ） ; the proportions of CD3 ^＋ cells and CD4 ^＋ cells in breast cancer group decreased significantly （P = 0. 0021, P = 0. 000 2） ; there was no statistically significant difference in the proportions of CD8^＋ cells, CD3 ^- CD16^＋ CD56 ^＋ cells, and CD4 ^＋/CD8 ^＋ cells between the two groups （ P = 0. 413 6, P = 0. 800 8, P = 0. 413 6 ）, the proportions of static total T lymphocytes and inhibitory T lymphocytes decreased （ P = 0. 010 2, P = 0. 021 7 ）, the proportions of active total T lymphocytes, active B lymphocytes, cytotoxic T lymphoeytes, and regulatory T lymphoeytes increased （P = 0. 000 0, P = 0. 000 6, P = 0. 000 0, P = 0. 002 9） . Conclusion： Immune function of the patients with breast cancer decreased significantly, but they may still be in compensatory stage without deeply inhibition.
