电视胸腔镜肺癌手术与开胸手术淋巴结清扫的研究

摘　　要：目的探讨在早中期非小细胞肺癌手术中,电视胸腔镜(VATS)手术与传统开胸手术淋巴结清扫的效果。方法收集新疆医科大学附属肿瘤医院2010年1月—2012年12月接受手术治疗的非小细胞肺癌患者158例,采用电视胸腔镜手术患者75例(VATS组),传统开胸肺癌手术患者83例(传统开胸组),比较两组患者的淋巴结清扫总数,并分别比较两组患者各区域淋巴结的清扫个数。结果 VATS组患者清扫淋巴结为1 257枚,阳性淋巴结数136枚;传统开胸组患者清扫淋巴结1 499枚,阳性淋巴结数169枚,两组淋巴结清扫总数差异无统计学意义(P>0.05);左、右肺癌各区域淋巴结清扫差异无统计学意义(P>0.05)。结论对于早中期非小细胞肺癌,电视胸腔镜手术可以达到传统开胸手术纵隔淋巴结清扫的效果。Objective To explore whether Video-assisted thoracoscopy surgery （VATS）can achieve the same effect in mediastinal lymph node dissection in non-small cell lung cancer （NSCLC）surgery.Methods 158 patients with non-small cell lung cancer receiving surgery treatment in Tumor Hospital of Xinjiang Medical University were collected from January 2010 to December 2012,among whom 75 were operated with the help of horacoscopic lobectomy while another 83 with traditional thoracotomy lobectomy.Com-parison of two groups of patients were made in the total number of lymph node dissection and the number of regional lymph node cleaning.Results In thoracoscopic group 75 patients,dissection lymph nodes 1 257,positive lymph nodes 136;In thoracotomy group 83 patients,dissection lymph nodes 1499,positive lymph nodes 169.There was no significant difference between VATS and conventional thoracotomy in the total number of lymph nodes dissection （P ＆gt;0.05）;There was no significant difference between Left and Right lung cancer in regional lymph node dissection （P ＆gt;0.05）.Conclusion Compared with traditional thoracotomy,VATS can achieve the same effect in mediastinal lymph node dissection in the early and mid-dle stages non-small cell lung cancer.
