空腹血糖水平与子宫内膜病变的相关性研究

摘　　要：目的研究空腹血糖(FBG)水平与子宫内膜癌发生率的关系。方法根据FBG水平将371例子宫内膜病变患者分为4组:201例FBG<5.6 mmol·L-1者为正常组,113例5.6≤FBG<7.6 mmol·L-1者为轻度升高组,38例7.6≤FBG<10.0 mmol·L-1者为中度升高组,19例FBG≥10.0 mmol·L-1者为重度升高组。比较4组胆固醇(TC)和三酰甘油(TG)水平,并分析FBG水平与子宫内膜癌发生率的关系。结果随着FBG水平的增加,TC和TG水平呈增高趋势。正常组TC水平低于重度升高组(P<0.05),正常组TG水平高于中度升高组、重度升高组(P<0.01,P<0.05)。FBG水平与子宫内膜癌发生率呈正相关(r=0.79,P<0.05)。结论子宫内膜癌与2型糖尿病之间可能存在关联。FBG的升高可能存在于子宫内膜癌发生、发展的早期,故对FBG升高的患者,建议行子宫内膜检查。Objective To explore the relationship between the levels of fasting blood glucose （FBG） and the incidence of endometrial cancer. Methods A total of 371 patients were divided into four groups according to the levels of FBG： normal group （FBG〈5.6 mmol· L^-1, n=201 ）. mild elevation group （5.6≤FBG≤7.6 mmol.L-1, n=113）, moderate elevation group（7.6≤FBG≤10.0 mmvl· L^-1, n=38）, and severe elevation group （FBG≥10.0 mmol· L^-1,n=19）. The levels of total cholesterol （TC） and triacylglycerol （TG） were compared among the four groups. Moreover, the correlation between the levels of FBG and the incidence of endometrial cancer was anaIyzed. Results The levels of TC and TG showed a trend to increase with the increase in levels of FBG. Compared with normal group, TC levels increased in severe elevation group and TG levels decreased in moderate elevation group and severe elevation group （P〈0.01 or P〈0.05）. The incidence of endometrial cancer was positively correlated with the levels of FBG （r=0.79, P〈0.05）. Conclusion Endometrial cancer may be associated with type 2 diabetes. The increase in FBG levels may occur in early-stage endometrial cancer. Therefore, endometrium examination should be performed in patients with elevated FBG.
