摘  要:目的将类胰岛素样生长因子1(IGF-1)以明胶微球(GMs)为载体,置于兔下颌骨缺损处,从血清学方面观察其对兔下颌骨缺损修复的影响。方法乳化交联法制备IGF-1/GMs复合物,将27只成年日本大白兔根据数字随机表法随机分成3组,均构建下颌骨缺损模型,实验组缺损区植入IGF-1/GMS,对照组植入空白GMs,空白对照组不做任何处理。建模前后采静脉血,通过生化检查观察各组血液中钙、磷和碱性磷酸酶(ALP)含量的动态变化。结果建模成功,术后各组动物均未见明显的免疫排斥反应。术后各组ALP值均升高,实验组峰值明显高于对照组和空白组,差异具有统计学意义(P<0.05),且出现时间较早,维持一段时间后逐渐降低,并随着骨修复的完成逐渐恢复到术前水平;术后2周时各组钙值均呈现升高趋势,实验组高于对照组,差异具有统计学意义(P<0.05),4周开始钙值逐渐降低,实验组下降较对照组显著,差异具有统计学意义(P<0.05),到10周时趋于平稳,各组差异无统计学意义,而磷值与钙值相比,呈现相反的趋势。结论血清ALP、钙值和磷值在不同时间段的差异性变化,可以反映各组不同时期缺损处的成骨情况,表明IGF-1/GMs能促进颌骨缺损早期愈合。Objective To investigate the effects of insulin-like growth factor 1 (IGF-1), based on gelatin micro- spheres (GMs) as carriers, on serologic indices associated with new bone formation of mandible defect in rabbits. Methods Emulsifying crosslinked method was employed to prepare the IGF-1/GMs complex. Twenty-seven adult Japanese white rabbits were randomly assigned to 3 groups for construction of mandible defect models. The IGF-1/ GMs complex, blank GMs and nil material were implanted in the experimental group, negative control group and blank control group, respectively. Venous blood was drawn before and after modeling, which entailed dynamic moni- toring of the changes in blood calcium, phosphorus and alkaline phosphatase levels via biochemical tests. Results Following successful modeling, no significant immune rejection was found in all groups. The implantation resulted in heightened levels of ALP, which was culminated in the experimental group compared with negative control group and blank control group (both P〈0.05). This culmination of ALP was shown in the early stages and maintained for a period followed by a gradual reduction to the pre-implantation levels upon accomplishment of madible defect repairment. There was a trend towards an increase in serum calcium levels at week 2, in particular, in the experimental group (both P〈0.05). This was followed by a progressive decline that was more significant in the experimental group at week 4 (both P〈0.05). The among-group difference in serum calcium at week 10 was unremarkable. There was an opposite trend towards the ratio of serum phosphorus and calcium. Conclusion The differential responses in serum ALP, cal- cium and phosphorus at various stages signal the magnitude of bone formation, suggesting that IGF-1/GMs can pro- mote the early healing of madible defect.
