结核分枝杆菌 rpsl 和 rrs 基因突变与链霉素耐药关系研究

摘  要:目的:了解结核分枝杆菌 rpsl 和 rrs 基因突变的特征及其与耐链霉素的关系。方法对136例活动性肺结核患者痰标本进行菌型鉴定及结核分枝杆菌药敏试验,提取结核分枝杆菌 dna,应用 Pcr 扩增 rpsl 及 rrs 基因片段,并进行dna 序列分析。结果结核分枝杆菌药物敏感试验显示136株结核分枝杆菌中,44株耐链霉素,92株对链霉素敏感,耐链霉素率为32.3%。44株对链霉素耐药的菌株中,9株(20.45%,9/44)单耐链霉素,8株(18.18%,8/44)为多耐药,27株(61.36%,27/44)为耐多药。经测序分析,38株存在 rpsl 和(或)rrs 基因突变,其中21株(55.26%,21/38)为 rpsl基因单位点突变,17株(44.74%,17/38)为 rpsl 基因多位点突变。rpsl 多位点突变株中,1株(2.63%,1/38)联合rrs 基因突变。38株均存在 rpsl 基因 aaa121aaG 突变,其中15株(39.47%,15/38)rpsl 基因存在 aaG43aGG点突变,2株(5.26%,2/38)rpsl 基因存在 aaG88aGG 突变,1株(2.63%,1/38)rpsl 基因 aaG88aGG 突变联合 rrs基因 G1449a 位点突变,后1株突变形式及突变位点尚未见文献报道,为新的突变形式和位点。结论结核分枝杆菌对链霉素耐药与 rpsl 和 rrs 基因突变有关;耐链霉素结核分枝杆菌临床分离株 rpsl 基因 aaG88aGG 突变联合 rrs G1449a新突变位点的发现为进一步研究耐药机理以及耐药结核病的快速检测提供了依据。Objective To understand the relationship between gene mutations of rpsl and rrs and streptomycin-resistance in mycobacterium tuberculosis. Method A total of 136 mycobacterium tuberculosis clinical strains were collected and indentified from active pulmonary tuberculosis patients’ sputum and drug susceptibility tests were done; Extracted Mycobacterium tuberculosis DNA and amplified rpsl and rrs genes by PCR, then the PCR products were sequenced and analyzed. Result in this study, drug susceptibility tests of 136 clinical mycobacterium tuberculosis strains indicated 44(32.3%) streptomycin-resistant and 92 streptomycin-sensitive Mycobacterium tuberculosis. 9(20.45%,9/44) of 44 streptomycin-resistant strains were single streptomycin-resistant,8(18.18%,8/44) were poly-resistant. According to sequencing results, 38 isolates had mutations in rpsl and (or) rrs genes, 21 (55.26%, 21/38) showed rpsl mutation, 17(44.74%,17/38) showed multiple mutations, including 1(2.63%,1/38) rrs gene mutation strains simultaneously. the aaa121aaG mutation appeared in all 38 isolates, aaG43aGG mutation in 15(39.47%,15/38) and AAG88AGG mutation in 2(5.26%,2/38) in rpsl gene, and G1449A mutation occurred in 1 strain (2.63%, 1/38) in rrs gene, AAG88AGG mutation in rpsl gene and G1449A mutation in rrs gene have not been reported. Conclusion Mutations of rpsl and rrs genes are associated with streptomycin-resistance to mycobacterium tuberculosis. Two new mutation points in rpsl and rrs genes, respectively, have been identified form the study, which will provide new information for further rapid detection of drug resistant tuberculosis.
