15例腹腔镜下卵巢癌全面分期手术的配合和护理

摘　　要：目的探讨腹腔镜下早期卵巢癌全面分期手术的配合和护理的要点。方法选择河北医科大学第四医院2009年5月-2012年5月收治的早期卵巢癌患者35例,其中15例行腹腔镜下早期卵巢癌全面分期手术,另外20例行开腹早期卵巢癌全面分期术,进行回顾性对比分析研究。结果 15例早期卵巢癌腹腔镜下全面分期手术均顺利完成,无中转开腹,无感染、大出血等并发症发生,平均住院时间7d。手术前3d严格的肠道准备和手术前2h留置胃管是预防手术中肠胀气导致术视野暴露困难的关键,腹腔镜手术中应遵循严格的无瘤原则是防止切口部位转移的关键,超声刀等能量器械的必备是减少术中出血和术后淋巴囊肿的必要措施,手术后及早进行床上活动和早下地活动是防止肠粘连、肠梗阻和下肢静脉血栓的关键,也是术后护理的要点。结论医务人员手术娴熟的操作技巧、手术团队的紧密配合、加强围手术期护理是手术顺利完成的保障,开展腹腔镜下早期卵巢癌全面分期手术是一项安全、可行的新技术,不仅具有良好的治疗效果,而且丰富了腹腔镜手术治疗卵巢癌的护理理论。Objective To study the cooperation and nursing key points of the early staging laparoscopic ovarian cancer in comprehensive surgery. Methods Retrospectively contrast and analysis was made on 35 cases of early staging ovarian cancer patients during May 2009 to May 2012 in the hospital,in which 15 patients underwent the early staging laparoscopic ovarian cancer comprehensive surgery, the rest 20 patients underwent the open ovarian cancer comprehensive surgery. Results All 15 patients with ovarian cancer successfully completed the early staging laparoscopic ovarian cancer comprehensive surgery, none transferring open surgery, no complications such as infection, hemorrhage. The average time in hospital was 7 days. All patients accepted strict bowel preparation of 3 days and gastric tube of 2 hours before surgery, which was the key points to prevent the intestinal bloating. The principle of strict disease-free in laparoscopic surgery was the key to prevent the incision site transfer, energy equipments, such as ultrasonic knife, were necessary to reduce intraoperative bleeding and postoperative lymph cysts. After surgery, exercises on the bed and on the floor were essential to prevent intestinal adhesion, intestinal obstruction, and to lower limb venous thrombosis, also key points to the postoperative nursing. Conclusion The operation skill, the precision coordinates of the surgical teams and the strengthening perioperative nursing are securities of the operation completed smoothlyl The early staging laparoscopic ovarian cancer comprehensive surgery is a safe and feasible new technology, which not only has the good treatment effect but also enriches the caring theory of laparoscopic surgery in the treatment of ovarian cancer.
