肺原发性淋巴瘤的少见影像表现及病理对照分析

摘　　要：目的探讨原发性肺淋巴瘤的一些少见的CT表现。方法回顾性分析2005年1月至2010年12月经病理学证实的5例肺部原发淋巴瘤的一些少见的CT表现,并探讨这些少见的影像学表现的病理学基础。结果 5例均为双肺多发病灶,1例为双肺多发结节灶伴多发薄壁空腔,1例为两肺多发薄壁空腔伴有弥漫性间质性改变。3例为肺内节段性实变伴支气管扩张及病灶内空腔病灶,但空腔壁外缘显示不清。4例患者经治疗后空腔壁变薄或消失。结论肺内原发淋巴瘤的CT表现可以多种形态,以薄壁空腔样病灶出现的CT表现虽为少见,但仔细分析其周围病灶形态,结合病史,有助于正确诊断。Objective To investigate the unusual CT findings of primary pulmonary lymphoma and their relation to pathological features. Methods Five cases of primary pulmonary lymphoma confirmed pathologically from January 2005 to December 2010 were analyzed in relation to their pathological basis retrospectively. Results Among five cases, the CT findings included ： （ 1 ） One was multiple distributed cystic airspaces and multipl nodules on CT ; （2） One was multiple cystic airspaces, extensive bilateral areas of ground glass opacity ; （ 3 ） Three cases were consolidation with airbronchograms and the cavitations within masses on CT; After receiving chemotherapy, capsule walls of the cystic lesions turned thinner or some cystic airspaces diminuted and even disappeared. Conclusion Primary pulmonary lymphoma has various CT findings including thinner wall cavities which is helpful to diagnosis combine with clinic history.
