摘 要:目的比较白内障超声乳化吸除术后植入不同人工晶状体(IOL)眼与正常年轻人晶状体眼的高阶像差(HOAs)和调制传递函数(MTF)。方法回顾性病例系列研究。对于2012年4月至2013年1月在天津市眼科医院白内障中心行白内障超声乳化联合IOL植入术的80例(98只眼)老年性白内障患者按照植入人工晶状体的球差类型的不同分为3组:球面IOL HQ201HEP(I组)、零球差非球面IOL Akreos AO(Ⅱ组)和负球差非球面IOL Tecnis ZA9003(Ⅲ组)。对照组为正常年轻人眼(Ⅳ组)。术后3个月对各组行高阶像差和MTF的检查。结果全眼球差除Ⅲ组和Ⅳ组间差异无统计学意义(P〉0.05),其它各组间差异均有统计学意义(P〈0.05);全眼彗差正常年轻人眼最小,与人工晶状体眼比较差异有统计学意义(P〈0.05),而三组人工晶状体眼之间差异无统计学意义(P〉0.05);全眼总HOA除Ⅰ组和Ⅱ组间差异无统计学意义(P〉0.05),其它各组间差异均有统计学意义(P〈0.05);各空间频率下人工晶状体眼MTF(HOA)值均较正常年轻人眼低,差异有统计学意义(P〈0.05),三组人工晶状体眼中Ⅲ组MTF(HOA)值最高,Ⅱ组次之,Ⅰ组最低,差异均有统计学意义(P〈0.05)。结论非球面IOL植入术后明显降低白内障患者全眼球差,同时提高了患者术后的视觉质量,其中负球差非球面IOL作用更显著,但其与正常年轻人眼之间仍具有差异性。Objective To compare higher order aberrations (HOAs) and modulation transfer func- tion (MTF) with postoperative cataract phacoemulsification and intraocular lens implantation with dif- ferent IOLs to normal younger phakic eyes. Methods Base on spherical aberration of IOLs, the 80 cases (98 eyes) were divided into three groups: group I spherical IOLs HQ201t-IEP, group II aspher- ic IOL with zero aberration Akreos AO and group III aspheric IOLs with negative aberration Tecnis ZA9003. Normal young phakic eyes were group IV. Compute higher order aberrations and modula- tion transfer function of the patients of 3-month follow-up after cataract surgery and normal younger phakic eyes. Results Spherical aberration of four groups had statistically significant differences (P 〈 0.05), except group III and group IV. The difference of coma between intraocular lens and normal young phakic eyes was statistically significant (P 〈0.05) while the differences between the different intraocular lens were no statistically significant (P 〉0.05). Total HOAs of four groups had statistical- ly significant differences (P 〈0.05), except group I and group II. MTF(HOA) of eyes with intraocular lens was lower than normal young phakic eyes and the difference was statistically significant (P〈 0.0083). MTF(HOA) of the three groups with different intraocttlar lens also had statistically signifi- cant differences (P 〈0.0083). Conclusions Aspheric IOLs can significantly reduce the spherical aber- ration and improve postoperative visual quality of the patients, especially the aspheric IOLs with neg- ative spherical aberration. However, there are differences between aspheric IOLs and normal young phakic eyes.
