轻重硅油填充术后对角膜内皮细胞影响

摘　　要：目的观察玻璃体视网膜手术联合轻、重硅油填充术对角膜内皮的影响及二次手术取油后角膜内皮细胞数的变化。方法回顾性分析2010年6月至2012年3月住院需行玻璃体视网膜手术联合轻、重硅油玻璃体腔填充病人。共分析了198例，215只眼。应用方差分析或f检验对不同手术方式、手术前后不同时间角膜内皮细胞数变化进行统计学分析。结果玻璃体腔内注入轻、重硅油对角膜内皮影响术后第一天差异无统计学意义（t=0．425，P=0．08），P〉0．05。术后第一天与术前相比亦差异无统计学意义（t=0．932，P=0．06），P〉O．05。术后前房有硅油滴或乳化硅油，行硅油取出，二次术后角膜内皮细胞数与术前相比差异有统计学意义（t=24．885，P=0．001），P〈0．05。但轻、重硅油填充的眼之间角膜内皮数相比无统计学意义（t=1．436，P=0．071），P〉0．05。继发性青光眼患者术前角膜内皮细胞数与硅油填充前角膜内皮细胞数相比差异有统计学意义（t=34．256，P=0．000）。二次手术后重硅油填充眼与轻硅油填充眼相比有统计学意义（t=8．304，P=0．003，P〈0．05）。结论玻璃体视网膜手术联合轻、重硅油填充术，没有并发症时对角膜内皮几无影响，但二次取油术后角膜内皮细胞数均减少；硅油进入前房及高眼压均会损伤角膜内皮；重硅油对角膜内皮损伤是否较轻硅油小还需进一步观察。Objective To observe vitreous retinal surgery combined with light and heavy sili- cone oil filling effect on corneal endothelial and secondary operation and take the number of corne- al endothelial cells after oil change. Methods Retrospectively analyzed from June 2010 to March 2012 in our hospital who admitted line of vitreous retinal surgery combined with light and heavy sil- icone oil filled. A total of the 198 cases （215 eyes） were analyzed. Analysis of variance and t test was applied to corneal endothelial cell number changes due to the different operation ways, different time before and after operation for statistical analysis. Results There was no statistically differences on comeal endothelial the first day after vitreous retinal surgery combined with light and heavy sili- cone oil filled, and also no statistical difference after the first day compared with preoperative. There were statistically significant on corneal endothelial cells on postoperative anterior chamber sili- cone oil droplets or emulsified silicone oil, silicone oil and secondary postoperative compared with preoperative. But between light and heavy silicone oil filled eyes, corneal endothelial was no statisti- cal significance. There was statistically significant on the corneal endothelial cells number on second- ary glaucoma patients compared with silicon oil fill anterior chamber. Heavy silicone oil filled eyes after the second surgery was statistically significant compared with light silicone oil filled eyes. Conclusions There is no effect to corneal endothelial on those accept vitreous retinal surgery com- bined with light and heavy silicone oil filled surgery. However, the effect to corneal endothelial be- tween two kinds of oil needs further observation.
