摘 要:目的:探讨320排容积CT血管成像在肾脏肿瘤术前评估的应用价值及其对比螺旋CTA的优势。方法:对118例肾肿瘤患者术前行容积CT肾脏血管成像,观察肾动静脉及腔静脉。118例患者作为观察组,70例行320排螺旋CT肾脏血管成像的患者作为对照组,对比分析观察组和对照组的辐射剂量。结果:118例患者的双侧肾脏及肾动静脉均在容积CT的扫描范围内。7例肾静脉或(和)下腔静脉有癌栓;患侧出现27例多支肾动脉,9例肾动脉过早分支,2例肾动脉狭窄。观察组CTA辐射剂量比对照组明显降低(P<0.001)。结论:320排容积CT血管成像可精确提供双肾及肿瘤的血管情况,能为手术方式的选择和预防并发症的发生提供信息;较之螺旋CT成像,容积CT血管成像较大地降低了辐射剂量;具有一定的临床应用前景。Purpose: To study the renal vessel in renal tumors with 320-slice dynamic volume CT before operation and to compare the advantage of reducing radiation dose by volume CTA and spiral CTA. Meth- ods: Dynamic volume CTA was performed in 118 patients who were diagnosed clinically as renal tumors (renal cell carcinoma, renal pelvic carcinoma and angiomyolipoma). The variations of renal artery and the cancer thrombus of vein in affected side of renal tumors were observed on post-processing images, and the renal artery stenosis in bilateral sides were also observed. These 118 cases were selected as the observation group; 70 patients who had undergone 320-slice spiral CT renal angiography were selected as control group. The radiation dose was compared between these two groups. Result: Among these 118 cases, the number of cancer thrombus of vein, early branching of renal artery, and renal artery stenosis were 7, 27, 9 and 2, respectively. Mean radiation dose of the patients was significantly reduced from 28.31± 1.87 mSv in control group to 20.73±2.46 mSv in observation group (P〈0.001). Conclusion: 320-slice dynamic volume CT angiography imaging can provide precise information of the renal vessel, which can be used for the choice ofthe ways of operation and prevention of complications. Compared with the conventional spiral CT, volume CTA has the advantage of reducing the radiation dose but not lowering image quality. So volume CT has good clinical application prospects in terms of renal vascular imaging.
