摘 要:目的:探讨三种材料整铸支架式RPD基托的不同设计对基牙龈沟液中的AST,ALP水平的影响,为临床可摘局部义齿基托边缘位置的设计提供依据。方法:对牙列缺损患者采用镍铬合金、钴铬合金、纯钛三种材质的整铸支架式可摘局部义齿修复。同一修复体的左右同名基牙分别采用高基托对抗和龈缘暴露两种设计方式。分别在修复前,修复后1个月,3个月,6个月进行龈沟液内GCF量和AST,ALP总量的测定,分析其差异。结果:戴用义齿后1,3,6个月高基托对抗组和戴用义齿前相比,各项检测数据明显增高(P<0.05),差异有显著性。镍铬合金,钴铬合金,纯钛三组之间各个时期的检测数据差异无显著性(P>0.05)。结论:基牙龈缘暴露的设计方式与基牙龈缘高基托对抗相比,设计方式更趋合理,对牙周组织的影响更小。Objective To investigate the influence of different designs of base plate in casting framework RPD made by three materials to the levels of AST and ALP in gingival sulcus fluid,provide a evidence for the design of base plate in clinical removable partial dentures.Methods The defects of dentition were treated with three kinds of material cast frame removable partial denture:nickelchrome alloy,cobalt-chrome alloy,titanium.Two designs:high base resistance and gingival exposure were separately used in the left and right homonymous teeth in the same dental prosthetic restoration.Determinations of the GCF,AST and ALP in gingival crevicular fluid were performed separately on repairing anterior,1 month,3 months and 6 months after the repair.The data were analyzed.Results Compared with before wearing dentures,the test data of the group of high base resistance at 1,3 and 6 months after wearing dentures were significantly higher (P<0.05).The differences of test data in the groups of nickel-chromium alloys,cobalt-chromium alloys and titanium at each period had no significant.Conclusion Compared with the design of gingival margin high base resistance,design of gingival margin exposed more reasonable,less influence on periodontal tissue.
