摘 要:目的比较超声弹性成像(UE)中评分法及应变率比值法(SR)与常规超声(CUS)判断直径≤2.0cm的乳腺良恶性肿瘤的价值,评价SR判断直径≤2.0cm的乳腺肿瘤良恶性的最佳阈值及其诊断价值。方法分别应用CUS、UE检查乳腺肿瘤;以病理为金标准,绘制ROC曲线,求取SR最佳诊断阈值,比较CUS、UE评分及SR的鉴别诊断价值。结果CUS、UE评分法、SR法的曲线下面积分别为0.803±0.046、0.862±0.040和0.943±0.026;SR判断乳腺肿瘤良恶性的最佳阈值为3.23。结论 UE较CUS对鉴别直径≤2.0cm的良恶性乳腺肿瘤更具价值,且SR法较评分法更佳;以SR=3.23为最佳诊断阈值,判断乳腺肿瘤良恶性具有较高价值。Objective To compare the diagnosis value of ultrasound elastography (UE) scoring method, strain ratio (SR) with that of conventional ultrasound (CUS) for identification of malign or benign breast tumors with diameter ≤2.0 cm,and to observe the best cutoff point of SR. Methods All breast tumors were examined by CUS and UE. Taking pathologic diagnosis as gold standards and SR as the results, ROC curves were obtained to determine the best cutoff point of SR for differentiating malignant and benign breast tumors, and the diagnosis value of CUS, UE scoring methods and SR for identification of small breast tumors were compared. Results AUC of CUS, UE scoring method and SR was 0.803±0.046, 0.862±0.040 and 0.943±0.026, respectively. SR=3.23 was chosen as the best cutoff point. Conclusion The diagnosis value of UE is higher than CUS for identification of malignant or benign breast tumors with diameter ≤2.0 cm, and SR is better than UE scoring method.
