基于扶正解毒消积方的中西医结合方案治疗原发性肝癌的疗效分析

摘　　要：目的探讨基于扶正解毒消积方的中西医结合方案对原发性肝癌的临床疗效及预后的影响。方法将68例原发性肝癌患者分为中西医结合组(西医综合治疗+扶正解毒消积方加减)和西医治疗组(西医综合治疗)各34例,观察两组治疗前后肝功能、甲胎蛋白和凝血酶原活动度情况以及实体瘤疗效、肝功能Child-Pugh分级及生存率。结果治疗后中西医结合组患者血清白蛋白(ALB)水平较治疗前明显升高,且高于西医治疗组治疗后(P<0.05)。治疗后中西医结合组实体瘤缓解率(33.33%)和稳定率(87.88%)均高于西医治疗组的17.24%和62.68%(P<0.05)。两组治疗后Child-Pugh分级比较差异无统计学意义(P>0.05)。治疗后中西医结合组生存率为97.1%,西医治疗组为85.3%,两组比较差异无统计学意义(P>0.05)。结论基于扶正解毒消积方的中西医结合方案治疗能够控制原发性肝癌的复发和进展,提高患者的生存时间。Objective To research the efficacy and prognosis of integrative therapy based on Fuzheng Jiedu Xiaoji Formula （vital qi-supporting, toxin-resolving and accumulations-dispersing formula, FJXF） for primary liver cancer （PLC） . Methods Totally 68 cases of PLC were randomized into the integrative medicine group （ compre- hensive therapy ＋ modified FJXF） and western medicine group （ comprehensive therapy） , with 34 in each. The liver function, alpha-fetoprotein （AFP）, prothrombin activity, efficacy in solid tumors, Child-Pugh classification of severi- ty of liver disease and cumulative survival rate were observed before and after treatment. Results The level of ser- um albumin （ALB） was significantly increased in the integrative medicine group after treatment （ P 〈 0. 05 ） . The ALB level in the integrative medicine group was significantly higher than that in the western medicine group after treat- ment （P 〈0. 05） . The response rate i. e. 33.33% and stable rate i.e. 87.88% of solid tumor in the integrative medicine group were significantly higher than those i.e. 17.24% and 62.68% respectively in the western medicine group after treatment （P 〈 0. 05 ） . There was no significant difference between groups in Child-Pugh grades at each time point （P 〉 0. 05 ） . The survival rate in the integrative medicine group was 97.1% higher than 85.3% in the western medicine group, but the difference was not significant （P 〉 0. 05 ） . Conclusion Integrative therapy based on FJXF can control recurrence and progression of PLC and improve the survival rate.
