上颌前方牵引矫治骨性Ⅲ类错软硬组织A点和B点矢状向垂直向的变化及其相关性研究

摘　　要：目的:探讨上颌前方牵引矫治骨性Ⅲ类错软硬组织A点和B点矢状向垂直向的变化及其相关性。方法:选择20例骨性Ⅲ类错患者,采用上颌前方牵引结合快速扩弓进行矫治,对治疗前后的软硬组织头影测量值进行配对t检验、直线相关分析。结果:在矢状向上,硬组织点A与软组织点A(sA)前移的比例为1∶0.87,硬组织点B与软组织点B(sB)后退的比例为1∶0.96;在垂直向上,A与sA和B与sB分别下移的比例分别为1∶1.09和1∶1.21。A与sA(矢状向r=0.612,垂直向r=0.459,P<0.01)、B与sB(矢状向r=0.885,垂直向r=0.689,P<0.01)的移动呈现密切的正相关关系,唇部形态随着软硬组织A、B点的变化而趋于协调。结论:上颌前方牵引矫治骨性Ⅲ类错软硬组织A点和B点在矢状向垂直向的变化呈正相关,且下颌的反应较上颌更加明显。Objective：To analyze the sagittal and vertical correlation in the interrelationships of skeletal and soft tissue points A and B with skeletal Ⅲ malocclusion after maxillary protraction therapy.Methods：Twenty patients with skeletal Ⅲ malocclusion were selected and treated with protraction and RME in this study.Pre-and post-treatment variables were compared using paired t-test,and the relationship of soft and hard tissue variables was studied using Pearson correlation coefficient.Results：Sagittaly,the mean ratio of forward movement of point A with soft tissue point A （sA） and point B with soft tissue point B （sB） was 1：0.87 and 1：0.96,respectively.Verticaly,the mean ratio of downward movement of point A with sA and point B with sB was 1 ∶ 1.09 and 1 ∶ 1.21,respectively.Significant correlation was found between movement of point A and sA （sagittal r =0.612,vertical r =0.459,P ＜0.01） and point B and sB （sagittal r =0.885,vertical r =0.689,P ＜0.01）.The movement of the skeletal and soft tissue points A and B in addition to the lips movement,the tissue profile of the patient became more attractive.Conclusion：Nearly positive proportional changes in the skeletal and soft tissue points A and B existed with slightly larger response in the lower lip than the upper lip.
