摘 要:目的 分析乳腺癌组织中人表皮生长因子受体-2(human epidermal growth factor receptor-2,HER-2)基因扩增与HER-2/neu蛋白表达的一致性与相关性,探讨其与各临床病理特征的关系.方法 采用荧光原位杂交(fluorescent in situ hybridization,FISH)及免疫组织化学(immunohistochemistry,IHC)法分别检测164例浸润性乳腺癌患者的HER-2基因扩增与蛋白表达,分析二者检测结果的一致性、相关性及其临床病理特征的关系.结果 FISH与IHC检测结果符合率为59.75%,结果存在一致性(Kappa=0.099,P=0.008),且呈正相关(r=0.293,P=0.000).ER表达与HER-2基因扩增及蛋白表达呈负相关(r=-0.481,P=0.000;r=-0.172,P=0.028);PR表达及ER/PR状态与HER-2基因扩增呈负相关(r=-0.401,P=0.000;r=-0.382,P=0.000),与HER-2蛋白表达无关(P>0.05).另外,肿瘤大小与HER-2蛋白表达有关(r=0.189,P=0.015),与HER-2基因扩增状态无关;Ki-67增殖指数、患者年龄、淋巴结转移及肿瘤分级与HER-2基因扩增及蛋白表达均无关.结论 FISH与IHC两种检测结果存在一致性,但IHC(+~(卅))的患者均应以FISH检测作为评价HER-2基因是否扩增的标准.Purpose To investigate the correlation between human epidermal growth factor receptor-2 (HER-2) gene amplification and HER-2/neu protein expression in invasive breast cancer patients and its relationship to clinicopathological characteristics.Methods Using fluorescence in situ hybridization (FISH) and immunohistochemistry (IHC) method,the HER-2 gene amplification and HER-2/neu protein expression status were detected,respectively,and its concordance and correlation,as well as their relationships to the clinical characteristics were analyzed in a total of 164 samples of invasive breast cancer tissues.Results The overall coincidence rate of FISH and IHC was 59.75 % (Kappa =0.099,P =0.008),with positive correlation (r =0.293,P =0.000) between them.The expression of ER (estrogen receptor) was negatively correlated with HER-2 gene amplification and the expression of HER-2 protein (r =-0.481,P =0.000 and r =-0.172,P =0.028).The PR expression and ER/PR status was negatively correlated with HER-2 gene amplification (r =-0.401,P =0.000 and r =-0.382,P =0.000),but without relevant to the HER-2 protein expression (P >0.05).In addition,the tumor size was positive correlation with HER-2 protein expression (r =0.189,P =0.015),but not with HER-2 gene amplification.Ki-67,age,lymph node metastasis and tumor grade status were not correlated with the HER-2 gene amplification and its protein expression.Conclusion There is a concordance between the HER-2 gene amplification and its protein expression.HER-2 gene amplification status should be further detected by FISH in patients with HER-2 protein expression (+ ~ (井)).
