摘　　要：目的探讨经岩骨乙状窦前入路显微手术治疗岩斜区脑膜瘤的临床疗效。方法回顾性分析2010年9月至2013年9月经岩骨乙状窦前入路显微手术治疗的26例岩斜区脑膜瘤患者的临床资料。结果肿瘤全切除20例,近全切除4例,大部分切除2例。术后出现一过性失语6例、脑脊液耳漏2例,听力障碍6例,以及三叉神经、面神经和听神经症状加重4例,均经处理后好转。结论经岩骨乙状窦前入路治疗岩斜区脑膜瘤暴露充分,对颞叶和小脑牵拉轻微,损伤颅神经机会较小,有利于提高肿瘤切除程度和减轻术后并发症。Objective To explore the clinical curative effect of microsurgery through transpetrosal presigmoid approach on the petroclival meningiomas. Methods The clinical data of 26 patients＇ with petroclival meningiomas who underwent microsurgery through transpetrosal presigmoid approach from September, 2010 to September, 2013 were analyzed retrospectively. Results The total resection of the meningiomas was reached in 20 patients, subtotal resection in 4, partial resection in 2. Conclusions The petroclival meningiomas may be sufficiently exposed by the microsurgery through transpetrosl presigmoid approach, which may improve the extent of tumor resection and reduce the postoperative complications due to only slightly pulling the temporal lobe and cerebellum.
