活血法和止血法对早期非缺血型视网膜中央静脉阻塞的影响

摘　　要：目的观察活血通络药物及凉血止血药物对早期非缺血型视网膜中央静脉阻塞(Central Retinal Vein Occlusion,CRVO)视力和静脉循环时间的影响。方法 42例(42眼)临床确诊为早期非缺血型CRVO的患者(剔除转化为缺血型CRVO患者6例6眼)按早期使用活血化瘀中药还是凉血止血中药分为活血组和止血组。治疗至少14天以上,观察治疗前后视力和静脉循环时间。结果 (1)活血组转化为缺血型CRVO者2眼,止血组转化为缺血型CRVO者4眼,二者差异无统计学意义。(2)治疗后活血组视力由(3.935±0.79)提高至(4.418±0.491),止血组视力由(4.09±0.647)提高至(4.458±0.446),差异具有统计学意义。(3)治疗后活血组静脉循环时间由(26.154±8.793)秒缩短至(19.414±8.349)秒,差异有统计学意义,而止血组治疗后静脉循环时间由(24.633±19.019)秒缩短至(18.783±14.097)秒,差异无统计学意义。结论对于早期非缺血型CRVO患者,使用活血化瘀治疗可促进堵塞血管再通,取得不次于凉血止血药物的治疗效果。OBJECTIVE To observe the vision and vein circulation time of early non-ischemic central retinal vein occlusion （CRVO） with two different methods which was promoting blood circulation or cooling blood and hemostasis. METHODS According to the drugs of promoting blood circulation or drugs of cooling blood and hemostasis, 42 cases of early non-ischemic CRVO （6 cases was exclude because of translating into ischemic CRVO） were divided into Zhixue group and Huoxue group. The treatment period was at least 14 days. We observed the visual acuity and the vein circulation time. RESULTS 1. Two cases translated into ischemic CRVO in Huoxue group, and 4 cases translated into ischemic CRVO in Zhixue group, there was no difference between two groups. 2. The vision improved from （3.935 ±0.79）to （4.418±0.491 ）in Huoxue group, and from （4.09±0.647）to （4.458 ±0.446）in Zhixue group （P〈0.05）. 3. The vein circulation time was shorten from （26.154±8.793）to （19.414-±8.349）in Huoxue group （P〈0.05）, and from（24.633±19.019）to（ 18.783±14.097）in Zhixue group （P〉0.05）. CONCLUSIONS There was no difference between the two groups in the translating rate from the early non-ischemic CRVO to ischemic CRVO; the drugs of promoting blood circulation to remove meridian obstruction could improve the vision of the early non- ischemie CRVO just like the drugs of cooling blood and hemostasis, and it could promote congestion recanalization too.
